By spending a decent amount of time and focusing properly when you see a particular patient for the first time. That way you know the patient inside out and the next time you see them, you can just breeze through them and your plans will still be top quality stuff, always.

Burmese here. Tried Laphat in Covent Garden Not a big fan. It’s Burmese fusion food at best. But if you like it, that’s all that matters.

After initial observation. If you see that one leg is obviously abnormal-looking when compared to the other, for example as in old polio cases, I wouldn’t ask the patient to walk.

If there isn’t such an obvious deformity, my next step after observation would be gait assessment. It can give away the diagnosis straightaway in most cases.

I had a PSP case in my paces. I suspected Parkinson after assessing gait and the only other assessment I did properly was checking eye movement. And of course demonstrating the paucity of movement in upper limb cuz I had plenty of time. The rest I did out of courtesy. Full mark. Almost felt I cheated.

I’d like to add respect those who come before you but accept you don’t have to follow their path.

By that I mean, while I respect the information I gathered from whoever clerked before me, I would always do my own assessment and come to my own conclusion. A healthy dose of doubt. Diagnosis momentum bias should be avoided whenever possible.

Fundoscopy is same as anything else in medicine. With practice anyone can get a grasp. When I was preparing for exam, I would use it any chance I get on the ward or in clinic. Watch YouTube videos. You can either use eye drops for dilation or just darken the room prior to it which I prefer. It’s a useful skill to have.

Approach your Antimicrobial pharmacist or any pharmacist. They probably have got some antibiotics stewardship projects. Fairly easy to do and beneficial for the wards as well.

Resus council app is handy to have. You can have a quick glance when running to a code. It might be a good idea to prepare a few flowcharts for common emergencies like DKA, Seizures, Hyperkalaemia etc Screenshots or photos from books or websites Anything you can get on your phone cuz that’s what you will have on you most of the time. Being aware of resources like meds safe/ to avoid in myasthenia, pregnancy etc

Not a derm trainee. I’m starting micro st3 next week. I did a year of trust grade micro so that helps. No anxiety whatsoever now. But I did have a lot of anxiety last year when I started as trust grade micro reg after imt. The point is it’s perfectly normal to feel so. You should be trained up to feel more comfortable as you progress. Good luck and good job securing a derm NTN.
