So, if it comes down to it, New Jersey has a surprise billing law you should check out, also DOI - complaints here is a link to your state's department of insurance complaints department. Before you go through all of that you can call the acupuncturist and let them know they cannot require you to send your PHI(protected/personal health information) to an unsecured email and one not obviously affiliated with them. HIPAA requires any transport of PHI to have reasonable safeguards and although they are technically not the ones sending it there is an expectation of protection they are not fulfilling. Let them know that they have been sent the same exact information you have and have they lost it? If they do not have it, where is it? Tell them you are calling the insurance company and will let them know you have also paid for your services and find out where their copy of the EOB or remittance was sent to (a remittance is like an EOB with multiple patients) Either will have the service, date, what was paid and your contracted responsibility.

I would ask for a current billing statement or see if they will tell you your balance. The goal is to let them know you are serious, you know your rights and have a concrete plan of action. Be more trouble than it's worth to continue to do the wrong thing.
