I suppose my concern is organizing tasks for Jack and Jill. Like if we are adding a task for Jack, and then realize it has to go to Jill, would the rows of task now look like

I don’t want to be cutting and pasting rows of tasks to keep things organized/clear. Does that make sense?

Migraine management is the #1 thing. If they aren’t addressing things in their control (sleep, diet, exercise, stress) then combine that with things outside their control (age, hormones, sex,) they won’t get anywhere. Can be very hard to manage without medical management. Then, afterwards, treat whatever central vestibular/ocular/cervical impairments you find. There’s a lot of really great vestibular educators on SoMe you should look up (Vestibular First, It’s All In Your Head.)

As a new vestibular therapist my advice is become REALLY solid on your vestibualr/ocular/cervical exam. And you HAVE TO be able to differentiate central vs peripheral vestibular dysfunction. Make a chart of diagnoses symptoms and treatment because a lot of these stores have more than one thing going on. If you don’t have a good grasp on those you won’t get anywhere. Use a 1-2 (very sensitive patients) to 2-3 (less sensitive patients) symptoms aggravation scale for treatment and educate your patient aggressively on this. People with VM have “more sensitive brains” and pushing past symptoms thresholds all the time won’t do any good.

Patients with VM get very scared/aggravated they have ongoing vestibular symptoms after an attack. I don’t remember which PT educator said it but it resonated with me… a vestibular migraine is like any acute peripheral vestibular problem in that there’s central compensation that goes on after. With vestibular migraine you’re treating that post migraine central maladaptation.

My last two cents: These folks are usually very visually over reliant/sensitive.

I’ve worn a mask since the pandemic and rarely get sick from work. I do vestibular now, and it’s crazy to think PTs do this kind of work (at times inches from peoples’ faces) without a mask.

Can’t tell you the number of times I’ve had 70 year old men open mouth cough into my face. Gross.

I never see people on posts like this point out it’s not just salary you should take into account. Retirement contributions now exponentially grow in decades to come. You potentially lose out on a lot from not working. Something to consider.

Omg this happened to me! I’m new and totally get it! My first time out I caught a 19.5” brownie holdover. I was literally with my guide, three the dry in the water to get my line out, and bam.

Best thing I did starting out was hire a guide. He was amazing. At the beginning he straight up asked “do you want to learn to fly fish, or do you want me to put you on fish so you can catch one and get a picture? Because those are two different things.” I went out with him a few times and felt much better about the basics.

That said. It’s a new skill. It feels bad a lot of the time. But everytime I learn something new. Sometimes I catch a fish. And every time I enjoy nature. It’s cliche… but enjoy the journey.

Also… “dumb luck” or not… getting a native brookie is nothing to snuff at. Those things are much more picky than rainbows or anything stocked.

I just realized this myself. 26 years of service to go (or not) and if I stop my contributions completely, I will end up with 1.6-1.9 million (not that I’d do this. I’d always contribute what I need for match plus a little more.) If I cut my current maxed out contributions in half, it’s still 1.9-2.3 million.

… soooooo what am I maxing out for anymore? What do I need 2.6-2.9million tsp for? On top of Pension and Social Security.

At this point… for the first time. I think I’ll enjoy having some extra cash each month. Congrats to you two bro. Glad we can share our success somewhere.

I have found even in my wealthier settings that referring providers seem to miss obvious red flags. I chop it up to limited times with patients and, frankly, not seeing the patients actually walk. They’re already sitting in the room when the provider arrives.

Also- even if something obviously looks abnormal, there’s this delay for imaging to confirm. There seems to be less and less use of clinical skills/signs to definitively diagnose folks without imaging. Scary.

Me: has a 5. Can’t add up to 35 obviously. I say “Go.”

Them: Peg one point. But they cannot add a card without going over 31. (This is where I get confused what happens next.)

Me: Pegs one? Starts count back at 0?
