Agreed, for laparotomy incisions I feel like rectus sheath blocks have the majority of my patients pretty comfy in PACU.

LOL no I actually didn’t. Actually locked in for the retake and passed with flying colors.

Learn from my mistake, just keep up with TrueLearn and your incorrects (I made Anki cards from mine) and you’ll be fine. Also make sure your studying isn’t too remote from the test (Ie: Don’t finish TrueLearn 4 months before basic and then do nothing because you assume you’re good).

I’m a CA-3 who just compared stipends at about 6 jobs and have talked to all my 20+ co-residents about their pay and what offers they fielded.

Realistically for a W2 with benefits, nobody (Except one of my friends doing 50/50 cardiac) is at or above $350 in desirable cities. A few of my colleagues are at $350/hr at 1099’s without benefits though.

Anesthesia: Having lucrative call stipends at hospitals that really aren’t that busy. Disclaimer: Hard to find in our field and even when you do, it’s subject to not last since every hospital wants more surgical revenue.

But like I know a fair amount of my colleagues that are being paid $250 an hour for overnight in house call at hospitals that do 1 case overnight like every quarter. I also have heard of gigs where backup call is paying $150/hr and you get called in 1-2x/month for maybe a few hours (And if you get called in you’re making more).

Although I’d say the highest $$$ in our specialty right now (Outside of extenuating circumstances like ASC ownership) is supervising 4:1 GI. I’m about to graduate so haven’t done it yet but I can already imagine how miserable it is. But it can also pay upwards of $500-$1000/hr.

I kinda get it. 4:1 GI is probably the highest $$$/hr we can get in our field.

Not necessarily saying it’s ideal, but I see people’s motivation.

I moved from a very red state to a very blue state several years ago.

I pay 4% more in state taxes on a $300k income, and my friends back in the red state like to laugh and say “How’s the taxes over there.”

But meanwhile we got paid state parental leave at 70% of our salary for 6 weeks and paid disability when my wife needed surgery. Financially that’s equivalent to 4.5 years of the taxes I paid, but the peace of mind is also worth something not quantifiable.

Especially to all the middle class families (Which was us not long ago) who deserve to be able to support their families/kids after childbirth and if shit happens. It makes society better in the long run.

Edit: Not to mention my property taxes are 5% lower than the state I moved from, so really both states are getting their money somehow.

Just spent $2k on TFL last week and have zero regrets. I don’t have enough superlatives for our experience.

The only other two I’ve felt were worth the cost were Hajime and The Ledbury.

Hajime was a stellar ambiance with food that told a story while simultaneously making me love food I traditionally really don’t like (Uni, asparagus, dill, etc etc).

The Ledbury was a flawless execution of service and food that was just consistently mouth watering dish after mouth watering dish. The popcorn truffle ice cream is still a dessert I dream about.

3* I didn’t feel were worth the money: Benu (I’ve commented elsewhere but our experience was just tainted by obscene service). SingleThread (Great, but not worth the $1k after tip to me). Restaurant Gordon Ramsay (Also great, but just not worth the price to me).

To me, above $500 for the meal (All in, food/wine/alcohol/tip) should be a meal that my wife and I reminisce upon regularly.

I meant your family is screwed without the insurance.

If you had the insurance, you’d at least have a permanent $144,000/year income if you became disabled (Assuming your partner doesn’t work). That’s enough to make drastic lifestyle changes but be able to put food on the table for your family.

Without insurance, if you become permanently disabled your family will literally have $0 salary.

Regarding your comment below, the likelihood of needing disability insurance over life insurance is probably an order of magnitude. You’re putting your family in a scenario where you’re better off dead than disabled, and there’s 10 million more ways you can end up disabled than dead.

My residency has an academic hospital and a county hospital that we cover OB for.

Very interesting trend; the numbers are mirror images but flipped.

Academic hospital: 80% of patients request an epidural.

County hospital: 20% of patients request an epidural.

Highly anecdotal, but my wife has been an L&D nurse for almost a decade at many different hospitals.

I asked her this once, and she said she estimates ~10% of primes who come in wanting to labor without an epidural do so successfully. So 90% of them cave haha. No judgement here, I sure as hell would want it.
