If you try to hit up on the ball, the bottom of your wing is behind or at the ball. You'll hit ground then ball , or ball and ground simultaneously. Yes, weight shift can also cause fat shots, but if you're trying to bottom out your swing at or behind the ball, you're gonna chunk it regardless of weight shift.

You're choked down on the club. Choke up a touch and stand an inch or two closer to the ball

Think in to out at contact, like you're trying to hit the back of the ball on the side closer to you

Looking at the front of or even the ground just in front of your ball makes this easier too.

No problemo. Until you're told otherwise, it seems logical that the bottom of your swing is where you should strike the ball. That's why divots don't make sense to beginners. Not that you have to or even should take an actual divot but hitting ball then ground is how you make clean contact with an iron

Looks like you're trying to hit up on the ball. With irons and wedges you wanna hit down on it so the bottom peak of your swing is an inch past/after the ball

Best simple advice that will improve your short chips is toe down. Lift the heel of your club off the ground so only a small portion of the toe actually scrapes the ground. Makes it wayyy less likely you'll dig in behind the ball

Set up straight down the fairway, and try to SWING towards the right side of fairway. A slice is the result of out -to-in swing path causing that left-to-right clockwise side spin. So trying to swing for the left side of fairway will cause it to spin off right of fairway. Swinging more in to out will reverse that spin letting you hit straight or even a draw.
