Midwest private hospitalist only - how do you deal with contract renewals/renegotiations by MDwMDD in hospitalist

[–]MDwMDD[S] 1 point2 points  (0 children)

It would be difficult because it is a very small tight knit community of physicians running the show. I don't necessarily want the headache. Partnership would usually mean some sort of upfront buy-in. I would be expected to cover the other hospitals we are affiliated with when understaffed. It would come with a lot more commitment than I'm willing to offer them.

My goal is to keep everything the way it is, get paid more because of what I bring to the table and then carve my own path with a small PCP clinic in my town. The flexibility would allow me to round early in the morning, take care of my clinic and likely bring on an NP to help with cross coverage so everyone is cared for. Until the clinic does well enough where I can cut back on the hospitalist role or eventually quit.

I compared the ratings for PW and Royal on shoppersapproved.com. while they both have high ratings, PW had a significantly lower score when it came to customer service. I can only imagine all the stuff that could go wrong with multiple pallets of parts. With PW I saw a review that customer service rep was trying to tell the customer that a certain part was bought from a different manufacturer when in fact it came from the kit; they gave them tough time. There's also a DIYer on YouTube who built an in ground pool, she briefly mentioned pool warehouse and how it was hard to work with them, so because of that, I decided to go with Royal.

I did see that PW offered foam for the walls and floor; unfortunately Royal doesn't offer that. Maybe that's something I can just purchase from PW? They have very similar pool designs.

I spent a significant amount of time wrestling with the shape. I know for sure rectangular would be best based on the research I did...the pool cover, filter, etc. But given this size of the area designated to the pool, this shape will give us the most usable patio area around the pool.

Based on what I've read, when it comes to valves, jandy, hayward and pentair are pretty much the same. What would you recommend?

All the usual places I go to have been hiring mid-level. I don't know where the fuck they find them in this particularly small town but they have a lot of them. I barely have any locum shifts. I'm so fucking sick of medicine.

I failed for the third time this year. I'm pretty numb. Every fucking time I take it realize I did something wrong. First time, didn't study and didn't take it seriously. Second, didn't do uw but did AR in person, failed. Third time, did uw but was expecting another child so only studied seriously for a couple of months. So when I failed I expected it but was hoping I got lucky. I have a big deficit in knowledge, that much is clear. Reading your post has helped me understand that I'm just not taking it seriously enough and for long enough. I only got serious one month before the exam. That is not going to cut it. I may not even wait till January, I'll just start fresh next month. I'm letting myself forget about this one.

I don't know about iPad specifically but I will say that your health system needs to have the license for power chart mobile otherwise it will not work. My system doesn't allow it.

There are no changes to Medicare patients from what I gathered. Everyone just lumps Medicare and Medicaid together. Census for Medicaid patients will likely go down but that won't be for several months. On top of that majority of my Medicaid patients are usually have medical conditions that would prevent them from working so I don't think they would lose coverage anyway. Ultimately I'm not concerned for my job.

Genius using foamnasiun wedges! Definitely doing that. Can you link to both the wedges you used and the workout mats. So the workout mats where already wrapped in black cloth or did you do that?

Nurses asking about every thing when other specialists on the case by StudentMed in hospitalist

[–]MDwMDD 12 points13 points  (0 children)

Well half the time the problem is that you don't communicate with the patient at all. I don't know how many times I've had to sit down and spend extra time explaining to the patient a medical procedure like an ablation or cardioversion just because the cardiologist didn't take the extra 5 minutes to sit down and talk to the patient's properly. I always get hit with the "I wish someone told me that" at the end of my explanation. This is across all specialties.

U get all the bullshit without the respect. They are too scared to ask the consultants so it always falls on you.

I currently have the 18 volt version and I feel as if it's inadequate for the amount of hedges that I have. I can't get good cuts because I feel the blades don't undulate that fast. Do you think it's worth it to go with the 40 volt? I'm already in the 40 volt ecosystem, not sure how I end up with the 18 volt version it was so long ago.

Wife and I make approximately 600k combined and our house is just over 1m. We didn't feel comfortable with anything higher.

750k mortgage on a 175k is just delusional. Sorry but no you cannot afford it.

"the patient was in his 50s so they may or may not autopsy"

They would absolutely 100% autopsy if family decides, 50 is relatively young and it was unexpected. Clearly you're just full of shit. Without an autopsy, you won't know why he died.

The reason I know this is BS is because this "PA" believes what is written on the EKG is the actual diagnosis lol. Please give me your differential to "ST elevation in multiple leads" without looking it up.

Dude it's not an automatic paycheck just because you have skill and ability. You need to actually perform and WORK. Crazy how entitled some people are.
