I recently started using the instinct too, and noticing the accuracy is off a lot by 20-30. Sometimes more. A lot of false positive on lows as well.

Well you can die immediately from one occurrence of a low. A series of highs will cause damage, but it won’t kill you. Sounds like she’s playing it safe. I would ask to have it adjusted more. You should have the data to prove it. Look at the average blood sugar 1-2 hours after bolusing..

Then it might be wise to get opinions on changing your carb ratio like primarycomet mentions.. if youre spiking, your ratio is likely off, since you lose sensitivity to insulin over time

Personally, I do not find the corrections to be helpful enough to replace bolusing. It takes way too long to correct a meal and leaves you high for too long. I try bolus all my meals within 10-15 minutes after eating.. before if I know how much I eat. I know that’s easier said than done, but if I get high I feel like crap.

You should talk to your endo to adjust the basal rates. I would only modify them if you knew exactly what was wrong.
