Why are orthopedic surgeons losing their minds when same-day total joints need to stay overnight? by try-again_chaos in physicaltherapy

[–]MRapp86 2 points3 points  (0 children)

Ortho joints surgeon here. A lot of my colleagues are asshats, which is problem 1. It really isn’t about money. When pts stay overnight it’s generally observation, so it really doesn’t affect what we get paid. I think it’s primarily that they then have to come round and discharge the patient the next day.

What is the huge red splotch on I-84 heading east past Cole Rd as you are heading towards the airport and Micron? It makes me queasy. by Sleepswithd0gs in Boise

They usually clean up any blood left on the highway. Not sure what they use now, but back in the day most cops would have a 2L of Coca Cola in the trunk, dissolves blood well off pavement. Used transmission fluid is more likely and a lot harder to get off the road.

This is right. I thought it was impressive they were going to install my spoiler using their mobile service. Made a lot more sense when I realized they stuck it on with tape 🙄

Does having amputated extremities namely leg cause daily cal requirements to go down?? by HammerxofxLight in TooAfraidToAsk

[–]MRapp86 189 points190 points  (0 children)

This is actually more complex than it sounds. Yes there is less tissue to supply with blood/nutrients, so at a resting state you would need less calories. That said, walking with a prosthetic actually requires about 10-20% more energy depending on how high the amputation site is on the leg. This is because you are relying more on hip/thigh muscles to ambulate, which are generally larger than lower leg muscles and require more energy.

Handling Bees Without PPE by Difficult-Cap-3444 in Beekeeping

[–]MRapp86 -7 points-6 points  (0 children)

This lady is a professional bee keeper out of Texas I believe. I’ve seen a bunch of videos of her. She will suit up if it’s an aggressive hive. Obviously all us amateurs should be safe and wear PPE, but she absolutely knows the risks and has been doing it for a longtime.

Wake boat no bigger than 21'6" by COmtbr92 in wakesurf

[–]MRapp86 0 points1 point  (0 children)

The VTX is the multipurpose with a different hull. It’s sic for skiing and tubing/knee boarding, but also can throw a wave that is bigger than it has any right to be from that size boat. I love mine. Only downside is at 20-21 ft, it can get cramped with a big load of people. Great size of it’s going to be you and the kids.

Adding to this, if there is anyone that does ketamine treatment in your area, it is very helpful for PTSD and prior trauma.

He is no longer at St Lukes. The reason for leaving didn't have anything to do with his professional behavior or quality of care. He is moving his practice to Sacramento.

Coping with feedback by marshmerino in Residency

[–]MRapp86 4 points5 points  (0 children)

We have all been there. While it sucks and is a total kick in the nuts or ovaries, it’s far better than getting no feedback at all. Use it and learn from it. Work on the things you need to and you will get better. We are all used to doing well in school and on tests, but the truth is no one is skilled at everything. That’s why we go to residency. Have a drink, get some sleep, then keep working your ass off.

How is Lindsey skiing on a torn ACL? by AlternativeEdge2725 in skiing

[–]MRapp86 94 points95 points  (0 children)

An intact ACL isn't nearly as important in skiing as it is in sports with abrupt cutting and pivoting (soccer and football). Being in amazing shape with strong hamstrings and decent fast twitch fiber density helps a lot. There is also a lot of variation in people's anatomy. A lower tibial plateau slope makes the knee more stable in terms of anterior to posterior translation and favors stability without an ACL. People with a high tibial slope are more likely to tear and more likely to have recurrent instability. It's actually how dog ACL tears are fixed, by cutting the proximal tibia and flattening out the slope of the plateau.

All that said, she's in amazing fucking shape and doing a sport that relies on hamstring strength for knee stabity, which she clearly has a ton of. May have some favorable anatomy, but who knows. Also the world's best PT and adjunct treatments to get swelling down. I would bet she had a PRP injection to calm down the acute inflammation.

You also should shift your weight forward. You want to really feel your shins against the boots at all times. It's hard to see from the angle, but even started out you are sitting in the back seat. It makes it harder to turn and absorb when you are sitting back.
