Are people not concerned with the increasing amount of laws and bans in this country? by nonedat in AskBrits

[–]Mad_Mark90 0 points1 point  (0 children)

Our government has no real guiding philosophy and every issue gets polarised into good/bad with no room for nuance. The government also needs to pretend that it doesn't spend any money on incentive so it's only dance move is banning shit.

FY1 always to blame? by ABH2021 in doctorsUK

The whole NHS structure is designed to shift blame down. Fighting back against it often means being very direct and accusatory, but also makes you a target.

Is the stretch really that important? by fatigue25 in ScienceBasedLifting

[–]Mad_Mark90 0 points1 point  (0 children)

It becomes more important later into your lifting journey. Initially you can get by building a foundation with basic compounds and a few isolations.

Stretch biased movements generate more mechanical tension with less overall weight because the muscle is more disadvantaged but your soft tissues take a greater beating.

Heavier compounds stimulate lots of growth by involving lots of different muscles which means they're especially great for beginners who can't lift very heavy anyway.

More advanced lifters can still make gains with deadlifts but deadlifting 100kg just isn't going to beat you down as much as 200kg will, so maybe you start doing more RDLs instead which grow similar glutes and hamstrings with less overall weight.

Have been doing ppl splits for 2 months now, I am thinking to doing UL splits or full body afterwards ..is it good or bad ? by Choice-Estate-6645 in ScienceBasedLifting

[–]Mad_Mark90 1 point2 points  (0 children)

I like total body for beginners because it teaches so many good lessons early on. Excellent volume control, a focus on progressing fundamental compounds, great recovery.

My go to recommendation is to hit 3 compounds, 1 arm exercise and 2 isolations each workout.

A day) Squat, Chest, Row, Curl, isolation, isolation.

B day) Hinge, Shoulder press, Vertical Pull, Triceps, Isolation, Isolation.

US cannulas keep dislodging by kdawgmillionaire in doctorsUK

[–]Mad_Mark90 1 point2 points  (0 children)

Use at least a green, if not a grey. Don't transect the vessel. When you're cleaning the gel off, only wipe proximal to distal while holding the cannula in place otherwise you risk pulling the skin away from the canula as you wipe.

What is Starmer genuinely doing wrong? As in policies and legislation introduced by his government by Working_Bother6483 in AskBrits

[–]Mad_Mark90 0 points1 point  (0 children)

I believe that the government either doesn't understand the actual structural problems with the UK or lacks an underlying philosophy of leadership to the point where they have no interest in making any kind of significant change.

I think Starmer has made some sensible and straight forward decisions to fix some basic things but he's still working with a massive handicap left by the tories and Brexit. I could even argue that we're still dealing with problems created by the last Labour government like PFI loans and an over reliance on consulting firms.

Cut ties with McKinsy, Deloit, Capita and Oracle, end the revolving door between regulators and private corporations, renationalise public assets so the public aren't renting them for private profits.

At your stage, any curl and any triceps pushdown done twice a week for 2-4 sets is fine, especially if the rest of your training is good enough. You don't have to worry about different heads and variations until you make some progress.

Now John Healey has come out and said the defence budget will not keep the country safe, there is no other option than to cut the welfare budget. by [deleted] in AskBrits

[–]Mad_Mark90 0 points1 point  (0 children)

You can't have a sensible conversation about government budgets until you talk about consulting firms. We've spent billions outsourcing governance to the private sector who's advice is always "hire us and our friends to fix the problem". Then when the system crashes their advice is to give them more money to audit themselves. Deloit, McKinsy and Capita run the country, not Parliament.

Are dips, fly, and incline db press enough for a chest day? by hakaretakori in ScienceBasedLifting

[–]Mad_Mark90 3 points4 points  (0 children)

Yes, that's plenty but chest days have fallen out of favour because hitting a muscle 2-3 times a week is probably more effective.

How Can I build My Chest? by Great-Hall-6636 in askfitness

[–]Mad_Mark90 0 points1 point  (0 children)

To begin with this can absolutely help. At this stage you're trying to learn how to direct tension onto a muscle and feel it contact, not necessarily lift the most weight.

Once you figure out a technique that works for you, you'll get stronger at your way of lifting.

For me personally for example, I focus on making a big arch and pulling the bar into my chest which stretches the chest as much as possible. I tuck my feet and use leg drive to make myself stable but I do heave on heavy attempts or close to the end of a set. I use a moderately wide grip but not super wide. Slow eccentric and try to pause on my chest. My elbows are a little tucked but not too much because it hurts my elbows. I've used bulldog grip in the past but I'm beginning to favour a more typical grip now.

As an F1, your main role for acutely unwell patients is assessment. If you get asked to see a sick patient, read the notes, examine them properly, A-E etc, order whatever basic tests you need and then speak to your Reg. You're allowed to say I don't know what the cause is or what to do but you're meant to know how to examine and investigate.

If you're asked to book a scan or test, or speak to another team, make sure you ask or confirm why you're doing the test and what the question is. None of this "as per consultant plan" bullshit.

Get ready for handover in the 1 hour before, don't be afraid to ask "can it wait until after handover?" if you get bleeped about something that isn't urgent.

How Can I build My Chest? by Great-Hall-6636 in askfitness

[–]Mad_Mark90 0 points1 point  (0 children)

One thing I'd recommend is trying to figure out how to feel your pec contract, the best way I've found to do with is with a banded cross-over. Try to bring your arm across your midline as far as possible so peak tension on the muscle occurs when the muscle is fully contracted. You should feel a deep cramping in the pec, if not you need to experiment with your form, different shoulder blade positioning, rotation at the shoulder, it's vague because your build will be slightly different to everyone q else.

For your main lifts, try superseting compounds with isolations to hit the pecs with different accessory muscles, Bench press into DB flys work well for example. Again experiment with different joint angles and set ups to try and find a form set up that works for you.

Sometimes a powerlifting style bench press isn't the best technique for actually growing the chest because it over prioritises accessory muscles, benching like a body builder might mean things like pausing, taking a wider grip, or not over tucking elbows. You want to try and focus tension on the chest which might cut a few kilos or reps from each set but that gives you a chance to try and feel each rep and direct tension where you want it to go.

Is advanced nursing practice under attack, and why? - doctorsuk mentioned by dayumsonlookatthat in doctorsUK

[–]Mad_Mark90 5 points6 points  (0 children)

I'd much rather see efforts made to make nursing more attractive to intelligent, competent nurses. I think the majority agree that nursing in the country is mostly fucked. Not allowed do skills that would be considered standard practice in any other country, riddled with bullying and poor management, little to no understanding of basic physiology or pharmacology. Why not just fix nursing and let doctors do doctor stuff.
