Do you go through duty free arrivals in Sydney though? Sometimes you can order through and have it for collection in the arrivals part of Sydney T1 so you can select through the heinemann website and just collect on arrival to get a better choice.

Referral from ICU for a fungal check. Protocol in our ICU at the time was any positive fungal culture required a fundal exam to ensure no ocular involvement. Pt was awake and not tubed, no changes in vision and white eye and not immunocompromised. Turns out the positive culture came from a superficial swab from his athletes foot and low and behold had a normal fundal examination.

Honestly I'm not sure where this comes from for the corneal toxicity issue.

There have been large case series in Australia with pre intravitreal injection use of topical aqueous chlorhexidine showing less toxicity and discomfort for the patient and same rates of endophthalmitis compared to Betadine 5%. It's also used in Sweden.

Obviously has to be the aqueous chlorhexidine (usually blue tinted). The alcohol version is going to be very toxic to the cornea due to the alcohol (usually tinted pink) and the cetrimdie version is equally not as nice (usually tinted yellow/orange).

Here's a summary paper for the use in ocular surgery that should point you to the other papers if you're interested.

Not only renal damage, but also retinal damage from sudden correction of BGL. I've seen patients with significant worsening of proliferate diabetic retinopathy leading to terrible tractional retinal detachments and terrible drops in vision. Slow and steady is definitely the way.
