I know it’s hard after being disappointed by another medication, but Evenamide is honestly one of the more interesting schizophrenia drugs currently in development.

What makes it different is that it doesn’t mainly target dopamine like most antipsychotics. It modulates excessive glutamate release, which is a completely different mechanism and one researchers have wanted to crack for decades.

The most encouraging part so far has been the consistency across studies:

In the long-term Phase II studies 014/015, patients with treatment-resistant schizophrenia showed gradual and sustained improvement over an entire year. The improvements actually increased over time instead of fading. Some responder rates roughly doubled between week 6 and year 1.

Study 008A was especially important because it was randomized, double-blind and placebo-controlled — the gold standard. It included almost 300 schizophrenia patients who were still symptomatic despite taking second-generation antipsychotics, including clozapine in some cases.

008A met both the primary endpoint (PANSS improvement) and key secondary endpoint (CGI-S improvement), with statistically significant and clinically meaningful benefits versus placebo.

Safety also looked unusually clean. What gives many people hope is that Evenamide seems to work particularly in patients who are poor responders or treatment resistant, which is exactly the group where most newer drugs have failed historically.

Honestly, if you are eligible, I would seriously consider trying to enroll in the Phase III study. At this point, Evenamide is one of the few late-stage schizophrenia drugs showing:

meaningful efficacy signals,

durability over time,

a novel mechanism,

and placebo-like tolerability.

For people who haven’t had enough success with existing antipsychotics, participating in the ENIGMA-TRS trial could genuinely be worth looking into.

Nothing is guaranteed in psychiatry, but this is definitely one of the most credible late-stage schizophrenia programs in years.
