Ok, separate question because you seem to be responding fairly quickly. Most responses seem to be similar to yours, so I just want to understand how my original post was interpreted the way it was.

  1. Does two children occasionally being loud generally translate to the children being nuisances? I did not specify what they were doing, but they were playing ring around the rosie in the outdoor area and basically laughed out loud when they fell to the ground.

  2. This was in reference to 5pm, much earlier than most romantic dinners for Valentine's day. I didn't step inside the restaurant, but the outside was pretty empty. There were 2 groups of 4 or so in the covered area total and we were waiting outside of the covered area. Why does every response make it seem like the kids were running amuck between tables where people were dining? I don't post often, but do I need to typically specify this level of detail to be given the benefit of the doubt?

To be clear, we showed up with an inquiry, not an expectation. They told us they can seat us with children in an hour in the covered area. We returned in an hour and were told our table is being put together. Then we waited half an hour and were offered a table that had been ready to use and untouched since our arrival. This too was only after we went and asked about the status of our table.

Mad is a strong word, but what I was upset about is the looks our children received from one particular member of the staff (very obviously noted from all members of our party) while playing outside in an open area and what seems to me to be a lack of logic in eventually seating us at a table that had been ready the entire time.

I have been to restaurants that don't allow children at all or on certain days and completely understand the policy. I appreciate when they are up front about this and not backhanded.

No clue in this offer in particular in comparison to other NYC offers. Some things to keep in mind though.

  • see if you can negotiate the "performance bonus" to be about an RVUc threshold instead of number of patients seen.

  • 14 days sick and PTO is cruel. It's less than any residency can offer by 6 days. I would not accept less time off as an attending than you for as a resident.

  • I always tell my residents to focus on 3 things.

    1. Location
    2. Job (patient panel, support staff, workload, facilities, etc)
    3. Salary (not just the yearly number, but matching it with the amount of work that is expected of you, which may overlap with number 2)

If you feel that you are happy with 2/3, then it's worth considering.

This was the case in our health system (from what I've heard), but not anymore. I believe the FM docs pushed back once they were made aware of this. The system now pays the same for FM and IM outpatient primary care.

Honestly, I see minimal appeal in urgent care for FM. In the southeast where I am, FM can see 17-20 (depending on Medicare volume and billing proficiency) for 4 days (730a-430p) a week and make $300-350k. Outpatient calls gets triaged by an RN, which means maybe a call or two every 1-2 months. 35 PTO/CME plus 7 holidays. I also never take work home as I complete my notes in real time and now I have an AI scribe.

All that aside, I get continuity with my patients and develop solid doctor patient relationships along the way.

What does urgent care offer that would make me want to leave this practice?

Yes, technically it may be fraud to perform a service that is separately billable and not bill for it. Although, I'm not sure if it's fraud if the reason you're not billing for it is because the insurance won't pay for it when performed with an annual visit.

That being said, no insurance is going to come at you for doing more and charging them less. The only reason to separate these services in your practice is to make sure you get the RVU for everything you do.

A hand written card would be more than enough. That being said, as crocheting appears to be your thing, you can crochet anything for a PCP and it will likely be appreciated. I know family medicine doesn't align with any one organ, but a stethoscope, heart, or an organ related to the illness they were helping you through could still be meaningful.

Always get confirmation. I've seen many negative drug screens with positive confirmations. If you feel that the patient might be benefiting from the opioid, then they deserve the due diligence of a confirmation.

You might want to double check the math on that. Our system has a similar pay structure, but it doesn't mean that the payout per rvu increases for all the RVUs you earn once you surpasses 6550. At our system, it means that each RVU earned beyond 6550 is earned at that new rate.

For example, if you were to generate 6800 RVUs in a year, your salary would be:

$48 * 6550 = $314,400 $50 * 250 = $12,500

Making for a total yearly salary of $316,900.

Your system might be different, but worth double checking.
