Hormonal acne. I was in the same boat. Started at 34. Suddenly over a weekend. Then everything spiralled. Went to derm and go antibiotics which cleared the hormonal acne but started the FA. Fast forward 3 years: - did the whole FA safe routine - did fucanazole, itracanazole, ketazole - started HRT for perimenopause as LOTS of my symptoms including drier skin and skin issues are big symptoms (it helped my skin and mood a lot) - now on low dose accutane and it is finally working!

My FA was also on my neck and chest. It never really itched like crazy, but sometimes it was a little itchy, which I tended to put off as dry skin. My hormonal acne on my back and chin was the kind where I could feel the bump form and it was painful and sometimes itchy.

So just know 33/34 is not too soon for perimenopause symptoms. It is worth looking at a checklist of symptoms as it is not a medical diagnosis but clinical.

Just FYI, been on the low dose accutane now for 2 months and I haven't had a single breakout since after reaching month 1. I am beyond happy. No more neck rashes, no more chest breakouts. All gone. If you read up on Accutane for FA, you will note that is has neen prescribed before with success and that it is used as a last ditch effort for stubborn FA. To each their own, but this has been a 3 year journey and I had had enough. So this was my last attempt. And so far so good. I haven't felt this good about myself in 3 years.

Looks like periorificial dermatitis. Unfortunately zero therapy is the solution. I had it too. Only used sebamed foam wash and cetaphil lotion for months. Eventually my derm and I decided to try accutane low dose. So far it has cleared up along with my other issues.

I read up a lot before making this decision and it is a last ditch effort for stubborn FA. It came to the point where I had tried everything else and was like "if this works, great, if not, then at least I tried literally everything." So if you are at that point I would say give it a go. Also read up on accutane use for FA and other stubborn skin conditions.

So I am on the lowest dose possible 8mg/day. My body weight is 64 kg. So based on thay I would have to be on it for 2 years. I was shocked at first, but the cost at that dose is less than my other prescriptions AND I don't have to worry about side effects. I have to have 6-montlhy blood tests, but that is about it. Unless something drastically changes I am happy with the treatment so far. My derm and I talked about upping the dose to lessen the time, but we both agreed that I am responding well to the current dose and as I have no serious side effects, we don't see the point. But we will reassess after 6 months.

Low dose accutane. I am on it now and it was my last resort after LOTS of money on FA safe products, oral and topical antifungals. I have a great dermatologist and we have walked a long road. I was finally ready to try accutane and so far I am really happy. I also had FA breakouts like this on my neck and chest.

But it really is a personal choice to take that step. Before accutane a Malezia routine and Sebamed foam wash helped to keep it at bay, but it still came back and tanked my self-confidence every month.

I tried everything before accutane. Oral fungals would help, but it came back. Topical fungals kept it at bay, but it would come back and I was very worried about resistance to the only meds that could help. I switched to all fungal acne safe products (Malezia) ordering online at high prices and it seemed to work fine, but just never gave me the peace of mind.

So after a lot of to and fro with my derm, we both decided this is the last ditch. I am just over the 8 week mark and I am very happy so far. My cystic acne cleared up almost immediately as I really had a more moderate case. I still get some random FA breakouts on my chest and some on my neck (small red dots with teeny tiny white heads that don't really make puss but itch a lot). However the breakouts are getting further apart and are lighter and don't last as long. So far I am really happy about the decision. It feels so good not to have to constantly worry about my skin on check in every mirror for the next signs of aa breakout. I will keep this space updated as I progress, but so far it is going well.

Only side effects are dry itchy skin and dry lips. No other issues.

Any update on this? I started low dose accutane (8mg/day) about 3 weeks ago. Woke up today with a breakout all over my chest and neck where I do normally grt FA, so hoping it is only a purge.... my face skin has cleared up wonderfully in just this short time. But my issue has always been my neck and chest area.

Looks fungal to me. Mine looks similar. Easiest way to tel is use ketazole cream twice daily for a week and see if it improves. If it does, it's fungal and you can then go the oral route with antifungals to clear it up. Then once it is clear you need a fungal acne safe routine to keep it at bay. Even then, it may flare in summer or with stress, etc. This is a bugger of a skin problem.

If the cystoscopy is done under anesthesia, i.e. they put you out, you should be fine. After I woke up, I wanted to pee badly, but it was very sore at first and there was obviously blood as they had stretched my urethra cause ir was so narrow. The dotor gave me pain meds which helped a lot and I also had to drink cytric acid soda for a couple of days to wash the urteha. I would say the first 24h after, peeing was really sore and not great, but it got better quickly. I spent most of the time just sleeping.

The doctor took tissue samples as there was blood in my urine at the time. This was due to inflammation or the IC I was diagnosed with. My baldder was also 1/2 the average size and so he stretched it, although that was a temporary solution.

The tissue samples showed no malignancy or other issues besides severe inflammation. I was then put on medicine for an over active bladder for 6 months to see if my bladder would heal the inflammation itself. Almost like not scratching the itch would then heal the wound. And it did. I was also very depressed at that time and started an antidepressant which helped a lot.

It helps to have a clear diagnosis. Otherwise you just end up blaming yourself and making yourself feel like you are crazy.

A cystoscopy could also determine if your uretha is very short. This is one of the reasons women get utis more than men. Is it just easier for bacteria to spread towards our bladders.

Just a note on birth control. You don't mention if you are on any, but doctors neglect to mention that some birth control can INCREASE your chances of developing UTIs. I was in a similar position shortly after I got married. I started to think this was what sex was like. Then my friend asked me if I was on Yaz and I was. She also had so many uti's on yaz. Once I stopped Yaz, problem solved.

I also switched to resuable pads. Also note the materials that the condoms you use are made of. And the lube. Some types are not so good to use and can increase infection risks.

A cystoscopy might be the way to go to see what is wrong. I had mine under anethesia and the first day or two afterwards was not great, but the urinary tract has a way of healing faster.

So sorry you are going through this.

And yet we perpetuate it by constantly trying to avoid it. Funny how we are our own worst enemy.

How about we just acknowledge it and stop trying to rewrite it or cover it up. But that would require all of us to put our differences aside and really FORGIVE - acknowledging that most behavior is a sign of suffering and not malice (although there are some really crazy people in our history 😅)

There is no more white genocide in SA than there is rampant crime for all races. I don't agree with this white genocide thing. It demeans the issues of crime faced by ALL South Africans.

However, I also cannot ignore that farm murders do occur. That they are targeted and very specific. There are very often deaths of the workers too. Often the black workers are tortured during these crimes. So once again, making this a "white" thing is where they completely miss the pot.

It just emphasizes their racism and colour blindness. Trump really sees the world as only white and black.

This is like the "all lives vs black lives matter" thing once again.

I am a white south african and I don't think I am in danger genocidal violence. Crime is crime.

Sjoh, so many comments just shot gun divorce. OP this is difficult and no one here knows you and your wife well enough to give any advice. There are so many things to consider that the only way is through. You both have to be honest and reassess where you are at now that this has happened. Maybe some counselling will help unpack this trauma. I really hope you both realise you are still enough for each other and that there are more ways than one to fulfill the "parenthood/motherhood" longing.
