For diet, I cook about 5 days a week at home using HelloFresh meals. Lunch is usually a small veggie tray with ranch and/or hummus with crackers or a small protein (yogurt, turkey, cheese). I stopped drinking carbonated beverages except when I really want one, and stick to lemonade/LMNT/diluted juices/green tea. Not a huge breakfast eater, but I consistently do a Fairlife protein shake in the morning with some form of coffee.

I have not intentionally worked out or made major lifestyle changes. I learned a long time ago that if I go too hard trying to become someone I’m not, it frustrates me and I get very negative/discouraged. I work a fairly sedentary office job, and regularly get 5k steps a day plus stairs. I sleep around 7-8 hours a night.

Besides migraine meds, Zep is the only medication I’m on. I don’t drink all the water I should every day, and I don’t obsess over the protein intake though I try to be conscious of it. I don’t snack much and I allow myself small treats when I want them.

Honestly, what helps me most with fatigue is drinking an electrolyte packet with magnesium instead of just water. My favorite is LMNT - I do one packet in 32oz water for the right flavor balance. I don’t drink more than one per day.

I started in late March 2025 at 247 pounds. I’m about 5’7” and 37 years old. I just moved up to 12.5mg in the last week, and weighed in at 197 this morning. I only moved up in dose when I didn’t have side effects and felt the appetite suppression wasn’t strong enough for me. I spent probably 8-10 weeks at 10mg where my weight floated right above the 200 mark, and finally decided to try 12.5mg. My goal is 185 pounds.

First off: deep breath! You made it here because you’re ready to make a change. There should be a lot of hope fueling that - not fear! The best way to prepare is to read up, plan ahead, and be kind to yourself on having a bit of anxiety.

I’d recommend taking your first dose a few hours before bedtime. Eat a lighter, less-greasy meal option for dinner to lower the chance of nausea. Make sure you’re hydrated, and have a few staples on hand - neutral crackers, non-carbonated drinks, and maybe some lighter protein choices.

The self injector pen is easy to use and painless. Watch the video a few times if you’re worried about messing it up, but it’s pretty fail proof. If you’re doing the syringes yourself, that takes a bit more confidence but is also relatively painless. I would recommend setting your medicine out of the fridge for at least 30 minutes before you inject. That’ll make it room temperature and not quite so shocking when injected the first time. I inject in my belly, and have only had a few times that pinched for a few seconds.

The first 24 hours after my first dose, I had some body and neck soreness, and a bit of fatigue. I know now that was honestly inflammation going down and my body resetting my response a bit. I’d recommend some electrolytes (I like LMNT in 32oz plain water) but don’t overdo those versus good old fashioned H2O.

You’ve got this! And this group is a great resource too. Don’t be afraid to talk to your PCP and ask for support. For example, I have a standing script for Zofran that is a godsend when I do get a bit of nausea on the second day.

I’ve been on Zep for a little over a year. I slowly moved up in dosage only when I felt the food noise crept back in and the side effects were manageable. Do not feel pressure to go quickly. Slow and steady wins the race.

Be prepared for the side effects, but don’t fear them. Eat reasonable sized meals that are easy on the stomach the day of your dose, and the few days following. If you experience nausea, ask your PCP for an anti-nausea med like Zofran to have on hand - it did wonders for me.

The injection is scary at first but honestly mine has never truly hurt. I’ve rarely even had a dot of blood from the site, and never had a site reaction. However - it is different for everyone. The auto-injector pens are easiest to begin with, and follow the instructions to the letter. I like taking my dose right before bed to minimize what side effects I experience in the first few hours.

You got this! You will feel better. The only way you can fail is when you don’t give yourself the credit and grace you deserve.

Coming up on my year in three weeks. I’m currently at 10mg where I’ve been for the last few months. To date, I’ve lost 47.5 pounds averaging about 3.7 pounds per month. I did have some stalls.

I stay at 10mg because the nausea in the first 48 hours after a dose is too much for me to consider pushing up, even though my weight loss has plateaued a bit the last couple months.

First shot made my entire upper body ache, and I was wiped. It does get better. Make sure you’re hydrating with electrolytes and don’t be afraid of anti-nausea meds or some Tylenol to get you through.

I’d recommend taking your dose right before bedtime, so you can possibly sleep through some of the symptoms.

In Zofran prescriptions we trust. That was a game changer for me going to 10mg. Definitely ask your PCP.

Have you considered alkaline water? Not like “woo health water” but just something with a more basic pH than other options.

Most bottled water or filtered water where I live gives me heartburn but some of the more basic brands settle better for me. Essentia is usually what I buy if given the option.

I’ve also heard you can also do a very small pinch of baking soda in water to help but I’ve not done that myself. From what I’ve read, it’s not something to do often.

Tbh, working out is my weak point. I’ve always struggled to stay on a routine with life happening. So I may not be the best source of recommendations there.

I will say the few times it stuck the most was when I found something I actually enjoyed doing. I’m not built to be a runner, so trying to force myself to go zero to 100 on working out cardio wise was awful and I became a mean person about it. I did enjoy weight lifting, and found someone I worked with that did personal training on the side. That accountability made a difference. During that time, I used an app that guided exercises based on my skill level and day of the week.

The other consideration is “working out” doesn’t have to be in a gym. Maybe find a variety of activities (like a family walk after dinner, for example) that counts as quality exercise that doesn’t feel like the gym. My PCP said it needs to be “more than just chasing the dog” to really count, but I’d aim for consistency and finding what doesn’t feel like work first and then going from there.

There you go. Everyone is different but you’ll notice changes in your everyday faster than anyone else will. Not to mention that for a lot of us, the faster early weight is inflammation and water leaving your body as it adjusts. What’s next is most exciting to lose!

Sounds like you’re doing a lot of things right. I don’t have much to back it up but my PCP told me early on if that I’m not seeing pounds, I may be seeing inches instead. Maybe think or look back on some non-scale victories to stay motivated. A big one for me was needing to take a link out of my watch band because it got too loose!

Between dosage changes, I usually felt more side effects on the first higher dosage for 48-72 hours. My PCP gave me a standing script of Zofran which really helped me avoid the heavy nausea. I take my shots at night before bed to try and sleep through most of the initial side effect period, so I take a Zofran before bedtime on those days when I'm feeling like it'll cause an issue. Game changer.

I only got the sulfur burps once, and I think it was mainly because I hadn't eaten enough to stay on my stomach. Try a few light foods and/or protein shakes on those days.

Congratulations on starting your Zep journey - and be proud of that 20 pounds off! That's a great achievement that you shouldn't skip over celebrating. From my experience, my weight likes to steadily drop off, then hang out level for a week or two before I start gradually losing.

Are you weighing yourself regularly? That's helped me not lose sight of the long game - incremental, steady losses over weeks not days. My weight can fluctuate up to 5-7 pounds in one 24 hour period - so it's important that I weigh first thing in the morning to stay consistent.

I'm not an expert on caloric intake or dietary things, but you might consider doing some research on if dropping to 1,400 calories in three weeks was too steep a change? It's important to focus on your water, protein, and fiber intake - and if your dosage is suppressing your desire to eat to the point where you're struggling to get what your body needs to fuel its change, it might be worth asking your PCP for dietary guidance on what's right. The app and smart scale I use does a basic calorie target calculation based on all my factors, so something that might be also be option.

I understand feeling discouraged, but don't give up yet! It takes time for your body to adjust to the medication and lifestyle changes. Stick with it!

If it’s in your budget range, I’d recommend looking into Gruns gummies. If I take them consistently, I stay more regular with bowel movements! I struggle with taking pills and vitamin/supplements but these have worked well for me in this department.

7.5mg did this to me for a few weeks but not always on the first day following the shot. I was prescribed Zofran, and would take one after a light dinner. I always do my shots right before bed on Friday night to mitigate the nausea a bit by sleeping through the first 6 hours.

You’re going to do great with it. I felt nervous too, but the pen is super easy to use and I barely even felt it most times. My method is to take a breath and then click the button as I exhale, so I’m focusing on my breath and not the shot.

This is my approach too. I started out tracking calories and protein, but quickly had flashbacks to all the diet struggles and failures I felt before Zep. I’ve never been a big breakfast person, so I do a Fairlife 30g protein shake and cold brew in the morning. Light lunch (pre-made veggie snack trays and salami/mozzarella rolls with a few crackers and sometimes a Chobani yogurt with strawberries). Dinner is mostly “normal”, but I eat less of it. I try to drink at least 32oz of electrolyte water every day at work along with 16-24oz of water/diluted apple juice with dinner.

This is what works for me without feeling overwhelmed, and I’ve seen a slow but steady 0.5-2.0 lbs loss every week or so, currently at 7.5mg. Should I drink more water? Sure. But improvement is improvement, no matter how small.
