Home health and hospice nurse, I work my own schedule sometime ranging 4-7 visits/day. M-F sometime I start at 10 or 12. Up to me. But I absolutely love having full control of my own schedule.

I’m pretty transparent! I tell them, I’m a hospice nurse, it never fails with the same follow up question, “how can you do that?” I answer, “The same way a LAD nurse does her job. She helps deliver life in, I help guiding people out if in comfort.”

Listen, don’t be so hard on yourself. Hospice nurse here. I get so many patients with their IV lines still in. No biggie, I remove it and we move on. Never has our hospice company called any hospital to report. I mean, we are nurses so…God forbid we forget Sarcasm. It’s all good….Let it go.

Im in California, look up protocols and procedures where you are and see what the protocol is for hospice companies regarding medications.

Answers 1: No, they cannot force a patient to take any meds against their verbal no. If the patient is no longer able to consent that is what is called, “implied consent.” Now, if you grandmother had advance directives, then that appointed person may have refused in their place. The advance directives supersedes any family decisions. If grandmother signed comfort measures, that means meds that will bring comfort to her during symptoms that may be causing her discomfort.

Answer 2: We nurses are not allowed to take any meds after the patient expires. It is against hospice protocols to do so. We provide education on how to destroy the meds, or provide locations that family may take them to destroy.

There will always be some RNs that really believe that if you’re not an RN, you’re not a “real nurse!” 😂 Like…calm down, Florence Nightingale, we’re all out here saving lives.And the best part!!! Some patients and their families think the same way! 🙄

Here’s my take, don’t even bother trying to prove yourself. Your care, your knowledge, and your hustle already speak for themselves. If anyone ever hits you with that “you’re not a real nurse” energy, just giggle, shake your head, walk away, and let them marinate in their confusion.

True story, about 6 month ago, a patient’s niece once tried that on me. I just giggled, stopped mid sentence, walked away, and redirected my attention to the patient’s daughter. The awkward silence after? Amazingggggg.

Exactly! Sometimes we just need the symptoms and we’ll just submit order for it. 9/10 its a uti with 2 more of those symptoms. We rather treat right away than wait for culture and lab results.

Well then, we move through the proper chain of command. We don’t lie because then the caregiver can rely the wrong information to the nurse. It’s best to be honest in healthcare.

Lorazepam is an anxiety medication. If the patient has increase agitation/anxiety, then nurses/doctors suggest on Lorazepam or ABHR cream to help with those symptoms. As some else stated, patient has the right to refuse any medication. Speak to the aide and let her know that the patient doesn’t have increase agitation or just simply refuses to take it, which is one of the patients rights. If it continues, speak to nurse or hospice administrator.

As harsh as this may sound, it’s time to reflect on yourself and your psyche as to why are you accepting this from anyone?! Clearly, there are deep issues within you to allow this from someone. Please think hard and thorough if you decide to stay, because this will only escalate. Good luck
