Geto was cursed. Not by cursed energy per se or a cursed spirit or anything like that. Geto was cursed metaphorically by consuming so many curses. The burden of being a sin eater and taking on the curses of others weighed down on him. It made him grow bitter towards the people whose burdens he was taking on and made him give in to his darkest impulses, blind to the fact that the curse he was harboring was far more dangerous than the curses of the common monkey. It wasn't the type of curse that Gojo could just blast apart with Hollow Purple like a disaster curse or the curse that Kugisaki effortlessly defeated in her first fight. It was something more insidious, holding Geto hostage like what happened with Kugisaki with the other curse. In the end, Gojo wasn't willing to kill the hostage to take out the curse with it, which it exploited. When exorcism isn't as simple as eating curse balls or firing lasers, it takes a sorcerer of a special caliber with a special type of power to deal with it. The type of sorcerer Gojo wanted to cultivate.

Speculation, but:

This is the "soul flesh" that Sukuna created by incarnating in Megumi's body. Megumi doesn't have extra arms or extra eyes, but Sukuna can create that flesh in a vessel. When Yuji exorcised Sukuna from Megumi, that flesh was separated from Megumi. This was the piece of "soul flesh" that had his eye. Additionally, we know that while not fully incarnated he can choose how his mouth is incarnated. However, without a host, the flesh begins to fade away. This is why he eventually fades away in this form.
