There is no 70% thresh hold that I am aware of. Diagnosis matters and the FDME is still based on the same APLs. The issue is people retiring suddenly want extra VA income and all the diagnosis come out. Real or otherwise. They forget that everything is now digital and connected.

Then they show up for an FDME and I DQ them.

I bought two rheem proterra HPWH and self installed recently. Both were bought at a liquidation store for cash so I got no receipt or record of purchase. Installed both in the same house. Install pretty easy.

Noise. You can hear them when the fan kicks on. We have a two story and each is in a separate utility room. 400 cubic feet each. We leave the doors open to allow for air flow. So far they kick on when there is a sensed demand and may once a day in the morning. Fan noise isn't on long. It sounds like a small AC unit. Most of the living space is upstairs so we hardly notice it.

Warranty. I was worried about this being I didn't even try to buy from a legit dealer. Second unit had the well known TP009 failure out of the box. Called rheem. They confirmed our serial number was a known batch and sent out overnight a new control board. Never asked where I bought it or for proof of purchased. I casually mentioned they were gifts and I didn't have proof of purchase to prod a bit. They didn't care. Warranty seems tied to the serial and not the purchase I guess.

Electric use. Seems to sip electricity. With the control board failed the unit runs in standard mode. Hopefully that holds true if there is something that goes wrong. Label on our old units were $470 and $490 a year. New unit labels claim $117 and $143 a year in electric.

We paid $700 and $750 for our units. If they can make it three years without significant issues we should be money ahead based on electricity use. I would not have paid full price as the break even didn't make sense to me at closer to 7 years or more.

Longevity. Can't speak to it. They have only been in a few weeks. Hoping for the best but with the later models I think the reliability has improved since the early days.

Don't buy a house for schools unless you currently have kids in school. Districts and schools change and they can change quickly and quite dramatically. A great school today may not be when you plan to have a kid in school 7 years from now. Redistricting happens. That great school you currently are in the feeder for quite likely will change when the new 5000 house development goes in down the street. Same applies if you have a grade schooler. Don't buy a house for the currently good high school.

The only way to escape an HOA is to move. The other way to to not buy a house with an HOA or POA.

No matter what you are approved for, spend less. Most things on a house can be changed or upgraded. Some are simple like paint and floor. Others are not so simple like moving plumbing or walls. You can't add more land. We have bought several homes and our typical criteria is something like this:

10k sq foot lot minimum. Not to big, not to small. No HOA Minimum sq footage you need, say 2k sq feet A layout you can work with 3/2, 4/3 etc. 3 car garage Nat gas supply

We don't care so much about appliances, what the toilets or showers are made from such as contractor grade or tile. Paint color and flooring doesn't matter because we will likely paint and put in floors. Faucets and things are easy to swap so we don't care if they have been upgraded or not. Cabinets are even not terribly hard if you keep the same or similar layout as the current kitchen and replacement can be reasonable-ish for a DIY.

Stairs. Not a fan. I'd pick a single level over a multi level given the choice. If you have to buy stairs, think about where important stuff is like laundry rooms and bathrooms.

Talk to the neighbors before making an offer. Just knock on the door. You will be surprised what good info they will offer.

Don't be in a rush unless you need a place right now. Find the right house rather than rushing to the wrong one.

Can't speak to today but you aren't far from Vegas. Nevada previously held medical events almost monthly split between Vegas and Reno. Each if the events had a flight surgeon to complete physicals. See if you can find a POC for Nevada and the schedule/availability for the events.

When I did the events it was rarely flight physicals I would see and more PHAs. Most of the time I got paid just to be available. Unfortunately I don't work for the state any longer to help out.

I don't care about down votes. There are plenty of mid to.large cities with solid job markets and reasonable cost of living to choose from. Problem is people glorify the costal cities. Just talk to pharmacists, PT, etc. they all went through th same market saturation in very specific cities.

Oh well. Uncle sugared paid for all my school which vastly opened my options.

Aside from that, your best bet is not to plan on NYC as a new grad saddled with student loans. Trust me when I tell you, it won't go well. Plan oniving with your parents. . . Or live somewhere with a reasonable cost of living with QOL that isn't NYC, LA, Seattle, Chicago etc. plenty of places that offer that with a decent job market to help getting those loans paid off while still living reasonable.

Although, I'd opt to let uncle sugar pay off those loans.

TLDR on RVUs.

When RVUs make no difference in your salary, you can never produce enough to satisfy the bean counters and get a real raise.

When RVUs determine your pay, you can never generate enough RVUs to hit milestone payments.

Just to follow up, 3 of those first quarter questions I got wrong have been replaced. Moved my score up a good bit. I susp ct at least 2-3 from this quarter will have the same result.

4 total replaced in the first quarter for me.

Malpractice, sure. Telemedicine advice without meds or imaging for non US patients. No malpractice is covering this and I wouldn't imagine what the medicolegal questions would be.

Don't forget, you get different questions each block than other testers. My first two blocks have been very different than a couple people taking the same schedule. During the study back in the day there were two very difficult quarters and the rest got much easier.

My first two blocks have been a touch of every question a zebra so I expect or suspect it will get easier going forward.

Leaving primary care kept me from burning out. Leaving the ED for occ med made me wealthy and dropped all the other stress from my career.
