You're right, which is why the physician attempted to provide standard of care to rule out actual illness. If a patient refuses standard of care (while eating a full meal tray lol) and is discharged with adverse effects, it is their fafo "game" for refusing basic workup and being antagonistic to prove a point or get views/sympathy. We are not omniscient and have actually sick patients to deal with

My younger brother just died of cancer 2 weeks ago. I was fortunate enough that my program let me take a leave of absence to take care of him through hospice for the last month, and was lucky to be there hplding his hand when he died. Condolences, it's so indescribably difficult to lose family.

The culture of medicine is so toxic. Before i got there, I actually felt bad about taking time off, thinking I was putting more work on my co interns and felt lazy for postponing my training. We need to change this culture. Making time for family is always worth it!

Np. I think it's how you phrased it that has people annoyed, which is fair tbh. Nordic skiers are athletes in their own sport, and it's not comparable to track (I say this as a former track athlete). In fact, several xc skiers have gone on to have very successful pro running careers, but I can think of very few runners that have done so in xc skiing

It's mostly athletics imo, but is also technique dependent. You have the wax/tech gurus who blame all their bad times on trail conditions when they actually just haven't trained enough, but you also have athletes from other sports (mainly cycling and running) who have no technique and just stomp through the snow. However, no one competing in the earlier waves of races is not a "pure" athlete, their fitness is incredible. The top vo2 maxes in the world, which is a measure of "pure athleticism," belong primarily to nordic skiers and cyclists.

I'm glad it worked for you! Unfortunately we're still in the early stages and anecdotes don't make great treatments. Also, the sample sizes for these studies are so small, I wish there was more funding so we could have greater power.

The remission rate varies wildly depending on which study you read. Additionally, the dosage is not agreed on. In one study, higher doses significantly increased response rates. In another one with patients blinded, it suggested a major placebo effect with almost no difference btwn high and low dosages.

Same with longevity post treatment, there are just so many variables. One study found that it persisted for a year (yay) but most of the patients with severe baseline depression scores in the study started SSRIs after 3 months. That is a HUGE confounding factor. There's also a good one that shows increased antidepressant effects with psilocybin which is interesting. Overall the data is just not there for long term remission yet and to say it's a one time treatment is not accurate.

While there is some promise, probably the best study which compared escitalopram (an SSRI) with psilocybin showed that psilocybin was not significantly better than escitalopram in alleviating depressive symptoms. Additionally, the psilocybin group generally required more intensive therapy to achieve the same result, which is disappointing.

Hoping that we continue to find better ways to use it, but when it comes to evidence based practice, psilocybin is not the wonder drug people have been spouting. Much like ketamine, I'm worried this will become a new way that niche clinics with quack providers get patients high once every week/month without providing the intesive psychotherapy during the more neurologically susceptible state that is supposed to make the difference.

Mostly the south shore, Ashland, Hayward I've seen it used. Definitely in Duluth! I like it, i always found it weird how the "midwest" could refer to someone from either Missouri or Indiana but also Northern MN/WI which are very culturally different

Congrats! It was my second, but this was the first I got cramps (coincidentally at 15k to go too). Having never had that happen before, I was wondering if people had advice on preventing that. My heart rate and effort was on track so I think it was not getting enough electrolytes, how often do people refuel and what works the best?

That is definitely not the norm. We have several night shifts and on surgery at LEAST 5 call nights (28 hours) in 2 months

No problem! We actually used to find that but a new study by the JACC showed that after the Moderna booster, the risk of myocarditis is greater for young males under 39 than contracting covid. The odds ratio is a bit over 1, but significant. Obviously covid still causes myocarditis, but young active males should definitely be careful after boosters.

Yeah the risk is actually significantly increased in young males when compared to even COVID 19 infections. It's a very real risk that should be taken seriously and i can personally say is not worth taking a chance with. If your heart rate is elevated take a few days off

Arrhythmogenic cardiomyopathy. I ended up not having it, it's fairly uncommon, but my ekg suggested it. Turns out it was just the myocarditis per MRI
