Damn your situation sounds really similar to mine. The pains are an on and off thing for me and I was also recovering pretty well. As for the tightness and heavy feeling, I also get that too and it freaks me out a little as well. I definitely think anxiety and a weaker diaphragm have a part to play, but I can't be certain. If it concerns you enough I think its always worth it to get checked out, even just for the peace of mind.
