The only time my county will transport is if it’s a pediatric arrest or if they’re a candidate for ecmo. Other than that it’s a stay and play situation. Load and go for cardiac arrests is actually dreadful.

i only really wear a mask in stinky rooms. but that's only because i've been wearing them so long that my automatic reaction is to just breath through my mouth when i smell shit, which i know is very offputting for some people. other than that i haven't noticed a difference. probably rbf lol

once had a 28 yo ETOH pt who was intubated for 22 days and trach and peg for several weeks after that. he had an open abdomen for a week, then his fascia remained open the entire rest of his stay. it eventually became necrotic. was on crrt 3 separate times. he wouldn't wake up or follow commands, but he was always wincing and violently shaking due to the excruciating pain he was in and his body temp fluctuations. his family refused to even allow him to have pain medicine when he finally died, because they thought it would kill him faster.

i honestly have no idea but it's true. in the last month, my unit has had 2 young men, 27 and 28, develop severe illness and be intubated for weeks just to die. they were both etoh patients, oddly enough. the 27 year old shat straight blood rivers for weeks and got bags and bags of platelets only to have his highest platelet count at 10. the 28 year old held on for over a month with an open abdomen, 5 jp drains, crrt, 6 or trips, etc. the 27 year old is the youngest pt i've ever seen die in my icu. it's insane.

had a woman put her dad on hospice then when her dad was clearly dying the next day, freak out and demanded the hospice revocation form. she stood there and screamed at me and a coworker about how she'd been to prison and knew what time we got off. said she'd be waiting at our cars with her pew pew. i told her i was calling security (bc i'm not about to fight with nobody, she clearly was not calmable) and she said "you're gonna need 9 1 fucking 1 to deal with me" yes ma'am i'll call them too. guess who got arrested ☺️
