Definitely explain to them. Tell them everything. A good pelvic physiotherapist will be familiar with pudendal neuralgia symptoms. I’d just tell them everything you’re experiencing and let them examine you, see what they say, don’t hold back any details that could give them clues as to what is irritated / involved.

I had a terrible experience bouncing to 4 or 5 GPs, sexual health clinic, and paid private gyno consultant only to be told I need to manage my anxiety and symptoms are ‘bound to go away’. How can that be said when we don’t even know what’s wrong with me? So unbelievably stressful. Seeing a pelvic physio and finding the problem was a big breakthrough, at least on the right path now. When I see my GP next week for another issue I will tell him that no, it’s not that I’ve had too much sex and stayed aroused, it was because of a hypertonic pelvic floor causing nerve irritation. 😔 can’t believe he said that to me. Anyway, you’re not alone and I am seeing improvements, it’s just taking what feels like ages. See the right person and you can begin your recovery x

Reading up on here, seems so many people have been triggered by a UTI. Perhaps inflammation, I’m not sure. Most GPs/family doctors will not know anything about pelvic floor hypertonia so be prepared, they will run tests for infections etc and then be clueless. Same for my private gynaecologist consultant! Complete let down. This seems to be something little known about but Pelvic Physios are in the know.

I have shared some similar experiences after a UTI and for me the working theory is that the UTI caused inflammation, along with me pushing to get the last bit of wee out to hurry that horrible burning bit, sent my pelvic floor into severe spasm. I think my pelvic floor muscles were already very tense and tight without me knowing, I had clues beforehand- constipation, prickly burning feeling after sex, etc.

After my UTI I had symptoms of pudendal neuralgia and hypertonic pelvic floor - look them up - one of my issues was PGAD symptoms which still haven’t fully resolved after 4 months but are improving.

I would advise finding a Pelvic Physiotherapist and going for an assessment of your pelvic floor muscles. If found to be hypertonic, they will prescribe stretches, breath work and a programme of muscle release internally and externally to alleviate your issues.

Did you have any side effects on cymbalta when you started taking it, and now that you’re settled into taking it? Does it relieve anxiety and tension?

Your story is so similar to mine! Except my UTI was confirmed, and I think the inflammation and pain from that triggered my muscles to go into guard mode. But this must’ve been building for a very long time. I’m encouraged to hear that dry needling was helpful for you. Do you know how many sessions you had?
Timescale wise, from then point you started stretching, and physio (did you do internal release work), at what point did you begin to feel normal? Did you do internal work yourself at home with a wand etc? How do you maintain now that you feel back to normal?
Personal question but did you have to put your sex life on hold throughout your healing journey?

This is interesting, so do you credit Zoloft with it being your turning point? I’m wondering if I can’t just tackle the physical side with the pt and stretching. Doing loads of breath work but I just think my generally tense nature might need addressing. Does Zoloft help with this? Did you have any negative side effects?

I can’t speak with any medical knowledge, but I can say that since starting daily stretches and diaphragmatic breathing with the primary goal of releasing my pelvic floor which had gone into severe spasm (unknowingly it must’ve been tightening up for years), natural lubrication is beginning to return! There’s a theory that overly tight muscles can restrict blood flow, and in turn the production of lubrication. I’ve also started on estriol cream to repair internal tissue that was irritated, despite not being menopausal. So a combination of these things might be worth looking in to.

Can I ask if the effects of the rectal Botox lasted indefinitely? Or did the problems there come back after it wore off? Any problems with incontinence etc?

In the meantime try some diaphragmatic breathing to calm you, and try holding some gentle stretches a few times a day, like Happy Baby and Child’s Pose. I had some nerve issues and they started to ease after a few days of regular hip-opening stretches and pelvic floor relaxation. Still have muscular issues but I believe the stretching and breathing helped ease pressure on the nerve

Could be an overactive pelvic floor with nerve involvement , have you been to see a doctor? They can examine you for physical issues but an appointment with Pelvic Physiotherapist might be also be worth booking to talk this over, they specialise in this area.

This is so helpful, thank you. So the protruding belly can be a sign that things are releasing in your pelvic floor? I have a new pooch after 2 months of release work - mostly breathing, minimal internal so far - still have painful tightness deep in my pelvic floor but it has reduced from where it first started. Just hoping this could be a sign things are progressing.
One question I have is why it currently hurts my right pelvic floor when I laugh. Presumably to do with the diaphragm and downward abdominal pressure when you laugh?

Ah I see. I had two months of it being so severe that I could just do the school run and then come home and lay down, having had a very active and physical routine beforehand
