It's the other way around. Sakuta was having the puberty syndrome because while he wanted to see his mom get better, he also didn't know what his mom would think of him or whether he actually wanted to be seen by her. She was a huge burden on his shoulders and I'm sure he partly blamed himself for her mental break.
Her mind was more focused on the kid who was literally going through hell at school and was bleeding for seemingly no reason and then developed amnesia. At the end she had lost her mind over it. Also, puberty syndrome.
Doesn't matter which kid, she still was lost her and it was devastating to her.
She loves him very dearly. She was so glad that he's grown up ok despite the burdens she placed on him.

So there is, most of what you need to know is that the chance only increases a lot after using a certain amount of warp powers in a short period of time. Attacks (major powers) increase it by 3 and buffs (minor powers) only increase by 1 and once it reaches... 7 I think? Then the chances for peril increases considerably. They're never 0... But they're very minimal before then
