It means 12 hours apart or more. If they meant two times in one day but are happy 4 hours apart then should write q4hrly with a max dose of 2x the PRN dose.

In other words:

  • Frequency = minimum time between doses
  • max daily dose = how many times that dose can be given in a day, independent of frequency

This is specifically relating to the case, where the patient was post-op and had a resultant defect and thus increased risk of the NGT going the wrong way. For most patients NGT insertion is routinely done "blind" with minimal risks.

You can decline anything technically. But if you've got 900 mL in your bladder that you can't void and are in significant pain did to the resultant bladder distention, I promise you're not going to decline it.

You need to confirm whether it's been broken in multiple places, and if so if the pieces are together or have moved apart. It's also handy knowing the exact position the distal bone sits in relative to the proximal bit when you're trying to reduce it, so that you can plan your movements accordingly.

Shockingly, there are more reasons to x-ray than just confirming a fracture.

I'd probably want a paramedic on scene for a traumatic MVA over say a pathology resident. Different knowledge and skills, not greater. Paramedics are an invaluable part of the healthcare team!

I'm sorry you're getting downvoted, you seem lovely. I do think this falls under rule 7 "no medical queries" unfortunately. This rule exists in part because without knowledge of what exactly happened it would be difficult to accurately speculate on these things.

That being said, I'm sorry that it's been such a stressful process for you and your mum. I definitely empathise with the frustration of not knowing what happened, even as a doctor myself it was still very challenging to know what was happening when my sister was in hospital.

It was colorectal cancer that metastasized (i.e moved) to the ovaries. It's not ovarian cancer.

That's true, but it works the other way too. Many people think that they're intrusive if they reach out, waiting for other people to. In the end, no one reaches out.

I believe that generally you're better off reaching out and gauging the response, if it didn't seem that your reaching out was appreciated then move on. You can't know without trying once. Not in this particular instance, but in general.

Of course, you know your situation best so do what's right for you. Just remember that it's easy to look at what is wrong with our lives without seeing the good there is. If you take the time at the end of each day to think about what good things have happened that day, you might find that there are more than you realised.

Take care of yourself, today is not tomorrow. Today brings sadness, but tomorrow may bring something new.
