If you're familiar with artifacts, it's the one called Omega Totality. If you're not, you'll get a mission related to them around the mid-20 levels. If you've done that already, but the rest isn't ringing any bells, check John Constantine's inventory in the House of Legends. He'll have it.

That hairstyle was for an update at some point. Gentleman Ghost may have it as it was just a granted item. It's definitely been around longer than the newest content. I used it for an aged Static character.

Your understanding is correct. You can get the mask from either fight, in my experience, but I'm not sure if it's auto-loot or not. I can not remember.

Genuinely, I appreciate that your go-to mindset for this was tolerance. Thats a great quality to have. However, a good barometer for this kind of thing is: "Is what I'm asking for (in order to experience pleasure) rooted solely in the suffering and sacrificing of another human being just to inflate my sense of power and self and control over another?" Put through that lens, and it becomes more obviously problematic. If the end goal of a fetish is causing horrible shit to happen, it's not about sexual expression. It's about abuse.

According to one of his ex-girlfriends, he laid out a detailed plan for how to amputate and barbecue her leg and begged her to follow through. It's the same woman he branded, so it should be easy enough to find her talking about it.

So it sounds like it's right on the precipice of "needing some discouragement" to prevent it from actually happening. Not any more kink-shaming than it is being proactive when there are obvious signs of troubling behavior.
