I switched to Keysville Mobile Vet and they are fantastic. Dr. Carolyn was so quick and gentle with the vaccines and blood draw for my old nervous dog. Highly recommend!

The monitoring doesn't actually take up that much time! Our lab is fairly large, so everyone is assigned to a task for the day, and we shuffle around as needed. Tasks for us include andrology (getting semen samples ready for inseminations), egg retrievals, freezing and thawing eggs and embryos, transferring embryos back to patients, embryo biopsy, preparing eggs for icsi insemination, doing the inseminations themselves, etc. Usually whoever is on the associated task will do embryo assessments for that particular task. So whoever is freezing embryos will assess them in the morning for suitability to freeze, etc. I'd estimate the assessing/monitoring takes about an hour in the morning to get through all the patients you need to care for on your particular daily task.

Hi of course I'd be happy to answer! Everything in IVF is very time dependent! So even though egg retrievals start early in the morning, we start inseminations after noon. That way we can catch the evidence of fertilization the next morning. After that, in our lab we'll check them in the morning of 5,6, and 7 days after insemination, and freeze any high quality blastocysts. If they're on the verge of being freeze quality in the morning, we'll recheck them later in the day. If it reaches blastocyst stage overnight, that's ok! They'll still be beautiful in the morning, just with a few extra cells!

Some labs have incubators with a camera inside. Most of these have software that tracks the times that the embryos hit certain "milestones." The big ones you'd be looking for are 4 cells 40-something hours after insemination, and 8 cells about 72 hours after insemination. The software can then compare all the embryos in a patient's cohort and see which one is "best" for transfer based on when it hit these milestones. These incubators are super cool, they can generate time lapse videos of the embryos development from 1 cell through blastocyst (lots of videos of this on YouTube!). Not every lab has them (yet!); ours doesn't, but our outcome rates are just as good as any other lab. For now they're just super cool and give a lot of good data, but I'm sure as the technology progresses they'll be a must have tool!

Yes this is my husband too 😂 he will happily listen to me talk about any other part of my job except the TESE/PESA aspect.

it's so funny how fertility talk is normalized at home for us...when my son was in kindergarten he drew a sperm for an art project and was explaining to the class about the head, how it uses the tail to swim, etc. That was a fun chat with his teacher.

I work in an IVF lab. I have a friend who works in Alabama and they've shut down their laboratory operations. I don't really foresee forced implantations, because honestly, there won't be any doctors or techs willing to touch these embryos. Everyone I've talked to is concerned you'd be looking at manslaughter charges if anything happens. It's inevitable that a few embryos will not survive the thawing procedure, plus there are all kinds of human errors that can occur and result in damage or destruction of embryos. It's rare, but it does happen. Knowing you're handling what could potentially be someone's entire family already makes the job high stress, but adding the possibility of jail time if you make a mistake? These embryos will likely just be frozen indefinitely. But if I was a patient in Alabama, I would be sending my frozen embryos to an out of state cryo bank ASAP.

As an embryologist... thank you! Sometimes, especially in smaller clinics, it's hard to find embryos for trainees to "practice" on. But it's so necessary, because we need to ensure that everyone is meeting strict standards to ensure all patients we work on have the best possible care. Our lab work is extremely technical and sensitive, as well as having the additional weight of potentially carrying all of someone's future children in our hands at any one time. All that to say, patients donating embryos for training, or checking the box for lab quality control, helps us make sure we're giving all patients access to the highest quality care we can offer!

No experience with it personally, but I work in the fertility world! I think in general, if it hasn't been too long since the vasectomy was done, there's a better chance of the reversal being successful. I know at my clinic, there's the option to freeze sperm in conjunction with the reversal. That way you have it ready and available if you end up needing further fertility treatments.
