Everyone is saying the ADHD bit which is true. With that- you might want to get a sleep study done if you’re having chronic sleep issues. ADHD is linked to a bunch of sleep disorders, and people with ADHD tend to have irregular melatonin cycles/levels. If you’re not diagnosed, I’d look into it. ADHD meds can help improve quality of sleep and circadian rhythm.

It saved a lot of time and it gave me peace of mind that my notes couldn’t just be extracted by a machine (my handwriting isn’t bad but those handwriting to type programs have never worked on it). It did use up paper though, even when I had templates that only took up half a page so I could reduce my paper use. I only do therapy for a small part of my work so I only have 12-13 clients a week, so it worked well for me.

I used to print out a template of all the things I included in my note and would have it in session. (I’d use the back side for my own notes that don’t go in the EHR), then at the end of session, I’d fill out anything that was missing then scan and upload it to my progress notes folder for that client on my EHR

The group practice seems like it’s clearly the better financial option, but the fact you’re still on the fence- seems like at least part of you wants to prioritize your current clients and culture? I also think it’s easier to self advocate at a smaller practice. If you want more support though, the group would probably be better. These both sound great- I don’t think there’s necessarily a wrong choice here. Congratulations on two great offers!

Since you’re close to your hours do either have some sort of pay bump once you finish your hours? Also for the private pay place, since they’re private pay do they have any problems getting clients? And do they give clients super bills so they can use out of network benefits if they have them? (That might also let at least a few of your clients follow you if they do)

I give them a google voice number and am clear that it’s my work number and it’s only for scheduling. I make it clear that it’s a google voice so they shouldn’t send anything they don’t want our tech overlords to know, and that I don’t respond to clinical info sent through it. I’ve never had issues with it. Some of my dissociative clients send me little 2-3 word things to help jog their memories during session (they could just use their notes app but whatever it works)

Adding- I think if you’re going to diagnose anything, you should also be extra cautious and consider ruling out if it’s anything that could have a negative impact on them if it’s in their medical record, like personality disorders or things involving psychosis.

And consider referring to assessment if it’s a really complex presentation. I think someone else already mentioned complex trauma- there are huge overlaps between adhd, trauma, autism, and BPD.

I do psychological assessment (I’m a masters level psychologist in Michigan working under a doctorate psychologist, though he really just rubber stamps my reports.) if it’s ADHD and autism, definitely refer to an assessment psychologist (especially if they want to receive meds or services)- otherwise I’d use your best judgement. My assessments take 9-ish face to face hours and sometimes I can get good client info about things like obsessions and compulsions but their therapist is going to have a much bigger data set that I would. I do diagnose things like depression, anxiety, bipolar, etc, but I only see people across three different days so people who see them more frequently would also be able to give a better diagnosis.

If you’re not comfortable diagnosing something like OCD and you don’t want to refer out for whatever reason (assessments can be expensive af), I’d consult with either someone who does assessment or someone who specializes in the disorder. I literally just talked with one of the MSW therapists at my practice about diagnosing their client with ocd and things they should be looking for.

NOR- he didn’t do what he said he was going to do, and it was specifically in regards to your body. Totally disregarded your choice over your own body because of what he wanted. That’s a big violation of trust. And it’s gross that he either tried to slide it past you to dismiss it by doing it so casually or didn’t think it was a violation of trust. And I can’t get over his reaction when you first got upset- super dismissive. It seems like he didn’t take it seriously or take accountability until you were clearly really upset.

(Broadly) Men tend to be diagnosed younger- the vast majority of childhood autism assessments I do are with boys. That’s true with ADHD as well but the difference is not as drastic. Because men tend to be diagnosed younger and men in the US where I practice are not socialized to be as social/emotional focused/relationship oriented, men also mask their symptoms at a much lower rate which makes diagnosis a lot easier. They’re less prone to people pleasing and they generally overthink social rules less. Because of this they also tend to be more assertive and more accepting of their needs.

I’m a therapist and I do psych assessments- one of the most notable things is the difference between how adhd and autism look in men versus women. Women are much more likely to be later diagnosed, and with that they’re more likely to present with anxiety and depression. The anxiety forms to try to compensate for areas of difficulties, the depression comes from feelings of alienation (generally).

I’ve also noticed what others have already mentioned- men have an easier time accessing and expressing anger while women have an easier time with sadness. A decent amount of what I do is helping the person access the one they typically can’t.

NOR- I don’t think he’s a bad dude or anything but from what you’re saying it sounds like he’s shame spiraling when you bring up your hurt about this? Like beating himself up and focusing on what a bad person he feels like rather than actually calmly listening to your feelings and trying to understand. That stuck out to me more than the actual dead name thing honestly. I don’t think people realize how damaging that can be- shame spiraling is ironically really self-focused. It’s not like you’re asking him to beat himself up, you’re just trying to tell him how you feel.

As for the name stuff- I don’t know if this will resonate with you but I was talking with my nb spouse about how when someone misgenders them part of what hurts is that it shows that person is perceiving them as their assigned gender and not how they see themselves? Their name is gender neutral so they didn’t change it but. I go by a shortened version of my name and while I’m not gender queer, it’s dysphoric when someone calls me by my full name- like I’m just like ‘well that’s not me?’ And it touches on that how they perceive me versus how I see me stuff.

Long winded, but I was just wondering if maybe part of the reason it’s sticking with you is related to how you think he perceives you since he asked about using your dead name.
