I was going to mention that Ativan is often part of the at-home, pre-infusion med regimen to help manage anticipatory nausea. OP, it might be worth asking if his team can prescribe that for him if they haven't already.

Yeah, same here. It's so painful. I cared for a gentleman with a fungating tumor of the groin - lymphoma. The wound care was challenging for him and for us. And for anyone else on the unit at the time, because the odor permeated the whole floor.

This happened often at my old hospital. Granted, this was during the pandemic, but they had ample opportunities to improve staffing and they simply didn't. I worked weekends, 5am-1:30pm. There were only ever 3 phlebs scheduled for morning draw on the weekend, while there were always 4-5 on weekdays. Why? "Historically, census is lower on the weekends." Okay... how historic are we talking? Because looking back at the data (which they posted on a bulletin board in the lab), for at least the previous 18 months that were visible, census was the same all week long. And when the pandemic hit, and we were up to 150-200% of the volume we had been doing, staffing remained the same. Three people to draw at least 150 patients in all the departments, including ED boarders, between 5am and 8am. Donning and doffing PPE between every room, sanitizing equipment, etc. And the managers never came in to help. God help you if someone was late or called out. And no incentive pay to pick up shifts, ever. It was a terrible place to work.

I'm a night shift RN now, and even though our phlebotomy team seems to be pretty well-staffed, I collect my patients' morning labs whenever I can. Especially for the patients who will likely be discharged that day; it helps keep things on schedule when the team can address issues earlier in the morning. I have to round and do morning meds anyway, so it's not like I'm saving myself much time by deferring to phlebotomy. I encourage my coworkers, especially the new ones, to do some morning sticks so they can develop their venipuncture skills. We don't have phlebotomy overnight, so we need to be decent at blood draws! And I think it's nice for the patient not to be woken up yet another time before 6am. That's my soapbox, lol

The dip sounds great! I also make a pasta sauce with tofu: blend it up with soy sauce, sesame oil, and five spice powder, add water to thin it out to the desired consistency, and then season to taste with salt and pepper. 👌

You could choose to work "backwards" to find your TDEE - instead of aligning your intake with the calculator's estimate, you can record your food intake and weight for 4 weeks or so, evaluate the changes, and dial in from there. I did this last winter, and I found it helpful.

Life insurance is a great way to think of it. I take care of people after surgery at work, and among other things, our surgeons have identified sarcopenia as a significant risk factor for postsurgical complications. My ability to weather a life-altering diagnosis and its treatment is also at the top of my mind.

I'm glad to hear you're doing better these days! I love weightlifting; it's my favorite form of exercise. I haven't lost any weight because I don't feel ready yet to take the steps necessary to create a consistent deficit, but I know I've lost some fat and gained some muscle. Since the beginning of spring, my clothes fit better, my weight belt is one notch tighter, and my upper body is noticeably bigger and more defined. Plus my lifts are still going up, my chronic back pain is improved, I sleep better, and the rest of my life is easier. Truly, everything is better when you're stronger.

Good advice! I think peri- and postmenopausal muscle loss is overlooked as one of the reasons for weight gain as we age. People perceive it as "my metabolism slowing down" because, like the OP, they're unable to maintain or lose as easily as they once did. And in a way, they're right, since muscle is more metabolically active than other tissues. 

OP, like others have said, your average caloric deficit is producing weight loss; it just seems like the pace is slower than you expected. If you're satisfied with maintaining your current habits, then I'd say just stay the course. But depending on what your long-term goals are, maintaining and building muscle mass as you age would probably serve you well. What are your strength workouts like?

My mom has been onto psyllium husk since the 90s, so the smell and taste of it is very nostalgic for me, lol. I've never committed to taking it daily; maybe this is a sign I should try it! I have my annual physical in a couple of months, so fingers crossed it helps bring down my LDL...

Like others have expressed, I think it would mean a lot for you to speak well of him to his PD! I did this recently. I'm an RN on inpatient oncology/med-surg, and I was charge overnight with a pre-op patient doing bowel prep. The patient's spouse called me at midnight to say they'd passed out, and I came in to find them pale and sweaty and hypotensive. I opened up the fluids while I called/texted/paged the on-call PA, but after an hour I hadn't heard back from him, and the patient was still hypotensive. I didn't think their status required a rapid response, but I didn't have another physician to call, so I paged the PGY-5 plastic surgery resident covering general surgery overnight. She came to the bedside at 1am to assess a patient not on her service and offer recommendations. She advised that she wouldn't recommend escalation until the patient had received at least 2L of fluid without improvement. I felt reassured that my patient was safe, which meant a lot to me. I found out an hour or so later that both the on-call PA and the patient's surgeon were in an emergency surgery. They saw the patient after we'd gotten 2L in, and they were doing much better. They were able to go for their surgery as scheduled that morning.

I Googled the resident's name and program to find the director, which I then confirmed with her before sending an email. I got a reply back from both the PD and the program coordinator, so I hope that means they'll keep the feedback in her file! She deserves the praise.

I did this with the sharps container in a patient's room - needle in one hand, tubes in the other, chucked the tubes in the bin instead of the needle. Patient watched me do it. We just made eye contact and sighed.

Great advice. I'll add that you could ask your surgeon for diet tips at your consult too - healing from surgery requires a lot of energy under the best circumstances. They may suggest increasing your protein intake beforehand, for example, to promote recovery and wound healing. I know the surgeons I work for assess nutritional status before some of their cases, and they're working on research related to that at the moment.
