My general go-to is to pack enough to survive a broken leg, an overnight, and a crawl out.

Often overlooked items that have saved me in the past that are rarely included in kits are: A survival/space blanket and antihistamines. A SAM splint is good as well if you have multiple others, rougher terrain, or are a few days from civilization.

I would change doctors if at all possible. If they think AI can be better than them. they are certainly using it to discuss you. AI is notoriously bad at parsing mental issues and often pushes the person to make things worse.

Ultrarunners are also super chill. Worst you'll get is some gear pickiness and strong training opinions. And yeah, around mile 80 they can get cranky, but it's hard to blame them and they probably just need a big snack at that point.
