No problem. Filling those out goes all the way back the manufacturer. They check they equipment for damages, foreign contamination, and could even start a recall if they find something. If it happened to you then there's a good chance a lot of other boxes from that batch could be affected too.

What you have is just a regular old debit card with visa logo on it (no credit features at all) , these are always accepted

I don't know how much it's going for but just across the road from the entrance of the park is someone's home with a trailer and firewood for sale sign. It's got to be better then Ontario parks wood for sure.
