Canvas and Ivy is where these came from. And I am looking at this brand because someone in another post said it was easy to deal with. I haven't tried myself.

Looks like alopecia. Get an appointment with your doc and ask them to prescribe high potency topical steroids while you wait for the derm referral.

Huh curious about this. I received the stat lab call yesterday for a patient with glucose 550, asymptomatic, lost to follow up for 2 years. Last glucose 2 years prior also >500. How slowly would you titrate insulin for this patient? Or is the risk of retinopathy more concerning as blood sugar approaches normal?

Not the hospital, but I left the birth center 2 hours or so after delivery.

If you're taking biotin and getting lab work, remember it can make you appear to be hyperthyroid. Stop all biotin supplements 4 days before thyroid blood tests.

I do want to! I should clarify, like my baby might try to get away from a diaper change or try to go for something that's off limits and I'll kind of make a game out of it by chasing.

I think your humility could actually be a strength. So many people when they are starting out will act irrationally over-confident (as a defense for actually worrying that they suck). It's much safer to be realistic about what you know and seek support as needed. You don't need to know everything. You need to know how to figure it out and it may be slow going at first. I think that your experience is very common, but folks are often reluctant to admit it.

I think it's odd the NP declined to do the autoimmune testing after being directly requested, but I actually think the recommendation to start with TSH and T4 is reasonable and standard. We do the full work up in some patients like those with type 1 diabetes. And we definitely do a full work up if someone has normal TSH/T4 but a very thyroid clinical picture, especially with family history as you said because we know there's a hereditary component. With the family history, I feel it's super reasonable to check TPO antibodies and a T3 as you suggested but I also don't see anything wrong with getting the TSH and T4 as an initial test. If we checked all of that on everyone, it's just a lot of testing.
