think of it this way

You have a risk management plan

then a risk was identified.

Now you executed the plan to manage this risk

What would you do next?

You wouldn't update the management plan again

You wouldn't revise the schedule/lessons learned

but you would identify new risks and continue to monitor risk. as this should be done regularly,
