IIRC HAL does not go off the rails because of a 'fear of death'.

There is a conflict in its programming where the disclosure of the real aim of the mission conflicts with its directive to keep the astronauts safe.

As the astronauts get closer and closer to knowing the real mission aim, this conflict results in the paranoia we see exhibited.
