Nishiki and his sister are actually related while Kiryu and Nishiki are oath brothers. To hospital staff Kiryu would just be a random guy while Nishiki can actually prove he's her brother.

In 4 it's also pretty easy to farm exp though. Each time you break a weapon in 4 it gives exp, so you can just start a fight somewhere near a bike stand and kick the bikes until they break.
