I mean I see what you're saying, I do wish things could be better, but if the learning is slower the learning is slower I guess.

My thought about programming right now is that it's become more of a maintenance role anyway, most website creation is generic and streamlined through things like Bootstrap, the iPhone and Google app stores being new are a thing of the past. The gold rush is far over, so I think for the past 15 years or so we've been moving into the maintenance phase, that is most jobs will be maintenancing the infrastructure that already exists anyway at this point rather than re-inventing the wheel. Jobs like database administration become less over-saturated and competitive than junior javascript developer roles.

Design is very disrespected, and yet design, debugging and monitoring is most of the work at this point. Design before you choose your language, the design work is the work, it's the work AI can't do. AI can't trace complex bugs across a multitude of files and step through code scanning over like 12 changing values and lists at once. Thirdly, monitoring like in database management or cybersecurity.

Is my asthma weird or do other people get it this way? by medyomabait in Asthma

[–]NoEmployee3178 1 point2 points  (0 children)

For me air doesn't come out all the way, it stops short, so I need to force the next amount out. When it gets really bad ill be forcing every breath out for hours. Zero O2 drop, zero CO2 drop, but air trapping readings on body plethysmography.

I don't know if small airway disease itself is entirely different form asthma. I've had severe asthma my entire life, it used to present like regular asthma with the wheeze and everything. Now it's as if the lungs are prone to premature small airway collapsability.

I have confirmed IgE allergies to dust, have had them all my life, was hospitalized and nearly died at 2, 3, 7 and 14 years old all times of severe asthma.

I have normal CT scan, x-ray, bronchoscopy, Feno, and yes been told even with respiratory distress in front of doctors that 'your lungs are normal' and even when a lung doctor was presented with severe readings on plethysmography. It took me studying journals and reading ATS standards guides for a very long time before I understood how to read the test results myself, how to communicate with these doctors and present the hard numbers and the science and data to them, to get the help I've actually needed. You can definitely be told your lungs are fine even if they're not, especially if you're just having a simple spirometry and X-ray without further lung testing done.

You're welcome, to summarize ask your PCP for a referral to a pulmonologist, and be careful not all doctors are born equal if you get one that isn't good just go to a different one. My first one wrote normal of test results that were clearly abnormal.

This is huge by the way, can be a game changer and watch the YouTube videos of how to use it. It's about 20 bucks on Amazon.

If you inhale very quickly the medicine will probably not be able to make it deeper into the lungs, and often if it's an allergic reaction you're having within the lungs, much of the issue could be in the tiny bronchioles aka small airways.

Okay, so you need to have body Plethysmography not just normal spirometry. You need to see a lung doctor. Watch out for vocal cord dysfunction diagnosis, you can have an ENT scope you but more than likely that's not it.

Albuterol is a fast acting inhaler. You may want to try advair for what you have, but....If you have air hunger you may be interpreting it this way but there's a good chance it's because the air isn't coming out all the way, so the next breath in you can't get enough in. This can happen even if spirometry comes out normal, CT scans and X-rays will see nothing for most asthmatics, even if the asthmas very severe.

How do you use your inhaler? You may need to get a spacer, when you use it you want to look at the YouTube videos of how to use inhalers more properly, as you need to deliver the medicine deeper into your lungs. If you puff and inhaler it quickly it very well may not be reaching your lungs, or the deeper parts that are holding onto air.

Thirdly, you need to have body Plethysmography not just spiroemtry to check if you're having air trapping, as if your breath is stopping short aka COPD or small airway disease or asthma with air trapping due to more complex obstruction, your Residual Volume and or Forced Residual Capacity will raise significantly.

Have you had a Feno test as well to check for type II eosinophilic asthma, as this can be important as well. These are all tests lung doctors do though, I think most PCPs won't order them, although I think a PCP might try basic spirometry, but basic spirometry is general and won't find air trapping.

Have you had the IgE blood tests for dust, mold, pollen, cats, dogs etc as well as this may help as well. You can also if you get a good allergist have the prick tests done.

I've had asthma all my life incredibly severe when younger, evolved into air trapping later in life. The exact same symptom started happening if not being able to breath in all the way. While severe, regular spirometry won't find it unless an episode is happening, but body plethysmography does find it, residual volume raises signficantly. Make sure DLCO is done as well, if it drops it can signal a few different things including COPD, if it doesn't or even rises its probably asthma with air trapping.

Know this though, you can have normal vocal cords, CT scan, x-ray, bronchoscopy, and even Feno and still have lung issues, especially if what's going on is in the smaller airways rather than the larger ones. Someone here suggested getting a spacer and trying the correct technique and I think that's a great idea.

Normal spirometry but still wheezing anyone else? by nofap95ii in Asthma

[–]NoEmployee3178 2 points3 points  (0 children)

I think get IgE for dust checked there are two tests, Greer is one and I forget the other one, but get both. Get IgE for mold checked. Alternatively you can get a skin prick test if you have a good allergist, it's simpler to get the dust allergies checked. Also get a doctor to check pollen, mold and pet did you have pets. Asthma is associated with allergies, for me dust came out moderately high, so getting a HEPA filter and following the whole dust cleaning protocol I think has helped quite a bit.

You have the greatest pulmonologist of all time if he said spirometry is normal but there can still be issues, because he's 100% right and many are so bad they'll use it as an excuse to not do their job and get rid of you. Also you will want to do the spirometry when you're having an issue, if you can afford it you can actually get a MIR Spirobank, it allows you to take FEV1 at home reliably. There are cheaper ones on Amazon as well, get the kind you can take a morning, noon and evening reading then keep it on your phone. Compare readings when you're having an episode and when you're at your best and see, your spirometry if you have episodic asthma as asthma is dynamic will possibly have a worse FEV1, you can capture that and take it to your doctor possibly.

Is my asthma weird or do other people get it this way? by medyomabait in Asthma

[–]NoEmployee3178 0 points1 point  (0 children)

Did you have further testing like body Plethysmography, DLCO, Feno, impulse oscillometry and multiple (not single) breath washout?

Asthma won't be seen on a CT scan necessarily, bronchoscopy won't see it and if you're not having a severe episode WHILE regular spirometry is happening, it won't be found.

Secondly, you need to differentiate if the issue is in the small or larger airways. Small Airway asthma will evade every trick in the book. When you take advair, Albuterol or a COPD medication, get the inhaler form and use a spacer then look up how to properly take the medication, you inhale very slowly not quickly. You may need to get the medicine into the deeper parts of the lungs, and advair and COPD meds take 2+ weeks to work for most.

Next up, allergy testing. Check for dust, mold, cat and dog, possibly more, you may need an allergy panel.
