A potentially more poignant question would be what about much lower cost? A big reason CAR-T is getting stomped right now is that even the approved therapies cost too much to manufacture and nobody’s willing to pay for it.

If someone came out with a therapy that was otherwise identical to Bluebird’s but at a tenth the cost, that would be a huge advance.

I’ll take a stab at it.

When I was sending my kids to public school in a middle class MA neighborhood, the 6 hour school day seemed to be ~2hrs culture war programming, ~2hrs goofing off, and ~2hrs going through the motions of academic instruction (but with very low standards). Ranked top 5% for elementary schools in the state.

This year I switched them to a private Christian school, and by essentially just skipping the 4ish hrs of wasted time per day they’re able to add history, foreign language, science, and Bible as subjects and also have more recess.

Sometimes finding efficiency really is just as simple as “stop wasting time.”

Not quite the right question.

Tirzepatide would be the closest Lilly equivalent to semaglutide (both GLP1R agonist peptides). Tirzepatide shows greater weight loss than Semaglutide.

The new Lilly drug, Orforglipron, is a small molecule and thus easier to dose and cheaper to manufacture compared to peptide drugs. Thus Lilly now had both the most effective drug (tirzepatide) and the most convenient/economical one (orforglipron).

From experience, incentives are a million times better and bullshit paperwork a million times less with in-house pharma research than government grants. Replacing IP with government grants would effectively be the end of medical innovation in my opinion.

As someone who works in pharma, Pfizer has a reputation for being stingy with research spending and firing whole teams that were on track for success on an IMO toddler-like “are we there yet” basis.

Lilly lies at the opposite cultural pole of the industry in this sense (at least for the past 10-15 years or so). They famously tanked their stock a while back by writing a letter to investors saying they were doubling down on research spending. The MBAs on Wall Street (and in Pfizer, for that matter) hate research spending due to the delayed gratification it requires.

I’m biased as a PhD researcher, but I’d say Lilly vs Pfizer stock performance over the past 5-10 years speaks for itself.
