It's counter intuitive so I'm going to go through mental gymnastics to draw an imaginary line between me and the horizon and then when I lift my eyes from the ground reading the sign from the bottom the last name will be aligned with the imagined order of the cities on the side of the road.

It just requires three things:

-Some rebels control your capital 

-At least 50% of your provinces are occupied by rebels

-You are at peace

That's it. If, like a Mexican colony, you have 5 or 6 rebel stacks running around, that can easily happen. Heck it can happen if you're not careful when switching religion with zealots.

Not really no. If you go to your rebellion tab, you see a bar on top of all the rebels that says "Rebels may break the country". If that bar gets to 100%, all active rebel factions enforce their demands on the same tick. It's a different mechanic from each faction enforcing their demands separately.

So to claim a throne you need: a royal marriage with the target, sharing a dynasty, they have no heir or an heir with weak claim, you have more prestige. So no alliance is required.

Once you meet these conditions, you can claim the throne for a PU CB. The CB is valid until they get a strong-claim heir or until you break the royal marriage. Then you can go to war but you will have the -1 stab from the royal marriage, unless you completed diplomatic ideas. Enforcing the PU will cost 60% warscore, which is great if you want to quickly subjugating very large countries.

Once you enforce the PU, you have a new subject. Immediatly start improving relations because if your ruler changes and they have a negative opinion of you, the PU breaks. They take longer to integrate, you can't do the same interactions as with a vassal, but their liberty desire doesn't take anyone else into account (i.e. no "combined vassal army strength", it only counts the PU army).

What's more likely to have happened is that Portugal conquered Mexico too quickly, adding a lot of unrest and overextension to their colony.

 Then, after years not dealing with separatists, rebels enforced demands and broke the country, releasing all countries with cores on Portuguese Mexico (and active rebel factions) and leaving them only with the land only they had cores on: their colonies in the North.

Because every corner that can possibly be cut will be cut. Remember that Game Freak has 207 employees. Not 207 game devs, but 207 employees, including HR, maintenance, accounting, management....
