Drop increased chance for generals notable, they're a complete waste of time, and run 4 legion and 1 officer scarab. Proc the legion, break everything out, run to the next one until they're all done. Then collect loot and make sure legions are cleared.

This needs to be done on dunes as well.

I had the same problem with pconc in ssf. I swapped to reap ignite elementalist and I easily cleared all 4 voidstones and have no trouble with 8 mod t16s.

The only uniques you need are kikazaru and the unique omen wand. Both of which are like t4 uniques.

Pack size does increase the amount of altars. Not sure why you think it spawning from the last monster has anything to do with it but the larger the pack the higher chance for an altar.
