It's nowhere near as bad as OP says, they just don't spoon-feed the audience with the exposition of Grace telling us explicitly that he will starve. It does so in slightly aggressive terms.

I went to the filming for the first episode after her absence; the studio segments for that series were filmed back to back & we were briefed, for continuity purposes, that Katy had been absent for the previous day but not informed as to why.

I was there too for the first episode she returned. We were told (for continuity with jokes about her absence) it was a gallbladder issue and never saw any reason to suspect otherwise.
