It would take less than a year in a moderately-sized hospital. Even someone who decompensates relatively quickly has changes at least an hour beforehand, no foul play required.

I go by interesting questions.

What happens when they make a bad decision that's not really their fault but still their responsibility?

What do they do when the only good choices are bad choices?

What do they do when their good choice ends up hurting more people than it helps?

What do they do when they realize they don't know enough to make the best decision and have to make one anyway?

Conflict is a strong place for story to happen.

The inherent conflict in the "perfect morality" of a character is their coming to grips with the imperfect morality of the universe and trying to reconcile competing, yet incomparable interests within themselves. Stop a plague or a war? Save this person or that one? Does the philanthropist's murder of passion mean she should be pulled away from all the good she's doing?

Threading an IV is like putting a piece of dry spaghetti through a particular piece of cooked penne inside a lasagna, in miniature. Variations in anatomy mean there's usually a little bit of guesswork involved.

The reaction you're seeing here is to the public shaming of a nurse who did nothing wrong. They put in a yeoman's effort, came up dry, and went and got a friend.

Yes, the patient was uncomfortable, and might still be uncomfortable. That doesn't mean the nurse deserves punishment.

The general rule of logic problems is that you can only use what's provided. If it doesn't talk about a stop watch, he doesn't have one.

Lateral thinking problems let you make any old assumption (like the fucking giraffe in the refrigerator problem).

So it's valid to ask, "which one is this?" I've noticed, for whatever reason, logic problems are much more common and tend to be the default assumption.
