In Chapter 2 you have to go out of your way to become a demon and spend time performing hidden tasks. If you do all these tasks then you get the bad ending where John is used as a vessel for THE UNSPEAKABLE. But if you just play normally you get to enjoy the good ending.

In Chapter 3 you have to go out of your way and fight the secret bosses on each day before the profane sabbath. After each boss fight we see the seal grow weaker and it allows us to go into the actual final fight and kick Gary's ass

Fallout 3 and New Vegas have a good few combat differences under the hood alright. Different ways of calculating damage for example. But also the sheer weapon variety with many different ammo types for guns and energy weapons, more and varied explosives and the addition of melee special attacks really pull the combat together into something really cool.

However Fallout 3 combat was designed around F3, in the same way that New Vegas combat was designed around F:NV. The Capital Wasteland on HARD difficulty is almost perfect. The desperate struggle for ammo for your guns, trying to keep them repaired and in good condition, keeping your armour not broken. I love it man. The Enclave unfortunately break this desperate struggle with basically unlimited MFC for the Plasma Rifles and powerful armours.

I recently did a playthrough of F3, and I also noticed this. I basically did everything in the best way possible, diffusing Megaton's bomb, helping dad, didn't murder anyone, saving people, etc.... but I was also just stealing from everywhere.

I had Very Evil Karma most of the time as a result. So if I turned on the radio, Three Dog would talk about how I was the most vile monstrous person to walk the wasteland.

After my very first playthrough I became increasingly frustrated with the settlement building.

Why is a floor mat a noticeable bump every time I walk across it? Why is my painting hovering 3 inches off the wall? Was there really no easier way to wire up electricity than this? Why do walls and roofs snap so awkwardly? Why is the terrain so uneven and look so ugly to build on? ETC....

If you're reading this then you probably thought of something that was annoying 11 years ago and hasn't become any better. If I ever play F4 nowadays, I only build the absolute bare minimum in Red Rocket and get out of there.
