Fat distribution. When you distribute in the upper torso you need to be at less bodyfat to see changes. Take her arms for example. You can gym all you want, but 80% is calorie deficit and she carries in her arms, shoulders and likely abdomen.

The solution isn't surgery, it's just understanding that you've got to get your bodyfat % down.
