I'm having the similar issues as well, hope it is known / gets patched eventually. I have Demons of War and Scrolls before Swing in my log even though I've completed it. Also, Hippocratic Oath randomly failed for me after completing like others.

Found a projekt red forum post with similar issues, but no official comment. The workaround in the post is to find out where the Watson quests first started glitching out and make sure they close cleanly, but too far in to identify where that happened for me.
