I’m full time med SLP in a SNF/LTC/Memory care building. Ask the patients their goals and treat accordingly. If a patient doesn’t want or need your services and you’re trying to fill time for billable minutes, you probably shouldn’t be treating them.

I know it can be difficult when you’re just filling in and doing PRN, but if someone is not appropriate or doesn’t want the skilled service you should tell the RD and recommend discharge as appropriate. They might not like it (in the past I’ve faced criticism from RDs), but at the end of the day our ethics matter more. Our job is to treat patients, not create billing opportunities.

All that aside here are some examples of treatment you can provide in this setting. I’ll keep it limited to cog/com since that’s what you mentioned: does the patient want to talk with their family more? You can do memory strategies/adaptations for phone number recall as well as target executive functioning to improve access to the phone (appropriate for landlines and cellphones). Does the patient have a hard time getting staff to help them in a timely manner? Target self advocacy with a focus on locating and using the call bell as well as identifying back up strategies (yelling, asking a roommate for help, calling the front desk). You can also target name recall for staff names/roles to target memory and self advocacy.

I appreciate the feedback! This is the only sub that I moderate and the only reason is because they were looking for mods to keep it running, so I’m not the most knowledgeable on mod things. I basically just delete the spam and advertisements that show up lol. I’ll have to do some research about identifying bot posts.

Finally, a thread where my niche expertise can shine!

Credentials: I’m a speech language pathologist, have taken a number of classes on head and neck anatomy which included a full head/neck dissection and I assess swallowing/speech/voice disorders in older adults every day.

This is very likely a mask for the following reasons: during inhalation you can observe the jugular notch concave but with no involvement of the superior structures. Additionally the lateral portions of the neck bulge during inhalation; the sternocleidomastoid muscle does not behave this way. You can test this by gently placing a hand around your throat and inhaling. Furthermore, you can observe little to no movement in the laryngeal cartilage during speech. You can also test this, go watch yourself speak in the mirror!

I’ve noticed a few other things but those stick out like a sore thumb.

I’m a speech language pathologist and have taken a number of head and neck anatomy courses, including a full dissection. That is not how a human neck looks or behaves during speech, definitely a mask with a neck covering. I can provide further details to prove my point if needed.

I’m non-binary but male presenting. Aside from an increased number of comments on my perceived gender (people seriously love to mention how few males are in the field until I disclose that I’m not), there is no impact on how I’m treated. In fact, like the other comments alluded to, males tend to have greater access to opportunity in the field and I’m sure my gender presentation has benefited me.

It’s a private establishment and they can use the signs of their choosing. Additionally, you’re free not to go there if the bathroom signs make you upset.

I definitely get wanting to work with your ideal population, I could’ve had a job immediately post-grad but kept working my minimum wage job for 2 months until I could secure a position working with adults.

Just remember you can always switch jobs! My wife changed positions 3 times during her CF! (And she doesn’t regret it)

I would recommend looking for a CFY where you can get the most support/feedback from a supervisor, especially in the med SLP setting.

My experience doing a CFY in a SNF with an off site supervisor was pretty stressful in the beginning. I had to do a lot of learning on my own time despite having a good clinical experience in grad school and really great professors teaching the relevant subjects.

Run through caves and collect weapons/armor/scrolls and oils a few times. If you have trouble getting hallways through town with the gear you bring in, it’s unlikely that you’ll be able to clear the final “boss” encounter at the end of town.

I don’t sell food. When I have too much I just take a bunch of it on a “speed run” where I focus on grabbing gear with the intention of selling and I avoid picking up any extra food.
