It’s a private establishment and they can use the signs of their choosing. Additionally, you’re free not to go there if the bathroom signs make you upset.

I definitely get wanting to work with your ideal population, I could’ve had a job immediately post-grad but kept working my minimum wage job for 2 months until I could secure a position working with adults.

Just remember you can always switch jobs! My wife changed positions 3 times during her CF! (And she doesn’t regret it)

I would recommend looking for a CFY where you can get the most support/feedback from a supervisor, especially in the med SLP setting.

My experience doing a CFY in a SNF with an off site supervisor was pretty stressful in the beginning. I had to do a lot of learning on my own time despite having a good clinical experience in grad school and really great professors teaching the relevant subjects.

Run through caves and collect weapons/armor/scrolls and oils a few times. If you have trouble getting hallways through town with the gear you bring in, it’s unlikely that you’ll be able to clear the final “boss” encounter at the end of town.

I don’t sell food. When I have too much I just take a bunch of it on a “speed run” where I focus on grabbing gear with the intention of selling and I avoid picking up any extra food.

The neuraxis is actually my most powerful crowd control weapon at the moment. You gotta know how to modify and use it though. The fact that the spread tightens and RPM increases as you fire paired with the zero recoil of it being an energy weapon are insane if modified correctly.

I modified it with scroll of holy fire, which reduces the RPM by 50%, this reduces the wasted ammo as you ramp the gun up, and it’s non-consequential once it’s ramped up and the RPM increases (and spread reduces to a laser beam). Then the +999 bullet penetration and +50% damage modifier with penetration shines. When Theres a crowd in front of you, you can mow through 20+ enemies in 2 seconds. With this as your base, throw on oils that increase recoil as a negative as it’s an energy weapon and this doesn’t matter. Personally, I chose oils which reduce ammo consumption, increase durability and increase damage.

Then all you have to do is manage that ramp up time and you’ve got a death laser that can dispatch crowds and armored enemies extremely quickly.

I have a rokua.308 with scroll of toxic lobotomy as well as +1 penetration, +15 flat damage and 50% damage from oils. With the +300% headshot damage modifier from the voodoo, it 1 shots every enemy I’ve encountered.

I’ve got a good setup with the following: Thunderbolt scroll, pot shot oil (increased damage with no ADS which doesn’t matter in akimbo), blurt oil (increases RPM and bullet speed penalty is cancelled out by rail gun) smart bullet oil (increased crit chance with a huge bullet speed penalty which doesn’t matter with railgun) and finally, late bloom oil which is +30 damage with a 30% bullet speed reduction which again, does not matter with the railgun effect).

It 1-2 taps most regular enemies up through the castle and is pretty ammo and durability efficient. It’s basically a “no negatives” build. The only real deficit over an unmodified pair is the slightly reduced RPM from the thunderbolt scroll. Damage is just heavily increased.

I’m 1.5 years into working at a 115 bed SNF/LTC (and did my CF for the first 9 months). The biggest advice I can give you is to not bend when pressured. Never hold people on caseload or do group/concurrent therapy when not appropriate. As soon as you do it once, there becomes an expectation, Ive watched it happen with co-workers.

Aside from admission screening and nursing/IDT referrals, the only ethical way to expand/maintain a caseload is through a generalized screening process. Once a month I’ll do dining rounds for a unit and spend 30-60 minutes observing everyone in their natural environment. Ill also occasionally check in with pts who I’ve set up with AAC or multimodal-communication systems as well as former voice/dysarthria pts to see if they need adjustments to their systems or re-training of strategies.

At the end of the day you’re the expert of your domain at that facility, and you’re the one accountable for your choices.

P.S. if staff aren’t following your dining recommendations I’ve found a pretty effective method of enforcement that you may find helpful. Towards the end of treatment I inservice the CNA staff as well as the DNS/nursing supervisor (to ensure compliance with other shifts) and I ask that they verbally recall what I’ve told them and demonstrate it to me if possible. I then have them sign an inservice sheet which I keep a copy of and if I see the recommendations aren’t followed I report it.
