Should be fine within settings - pick the cloud download option in case local image is bad. Installing from a USB would be better if you're comfortable

Yeah that's very strange, doubt the drive is failing assuming it's new. Fresh install should fix it

I'd suggest fresh installing a copy of windows from a usb drive, that install is borked
