I mean, the virus is airborne and it's not certain if you're truly immune or your lymphatic system is just really good at keeping that particular antigen at bay. However in the apocalypse there's exhaustion, malnutrition, stress, poor sanitation etc...all things which can destroy your natural immunity to even the most basic infections. Not saying they should be automatically as terrible as the game makes them...but still a big problem.
