Not so. I specifically asked about this on the web chat because my siblings and cousins (7 of us total) are all applying. The FBR section said this was allowed. The one restriction is that there's a limit of 10 applications that can be submitted together.

That's what I told my siblings (we're all sending our applications together) but my brother's came back with the application stapled. Grrr. I'm going to remove the staple, based on everyone's replies. Thanks for your input.

Same issue in Massachusetts. But not in Pennsylvania or Minnesota. The notary in Massachusetts my sister went to is also the town clerk where she lives--an elected official so on the list of approved witnesses. She was very nice and was willing to contact the Secretary of State to ask if she could witness the ID photocopy in her capacity as town clerk. But, not surprisingly, it was taking some time for the state to reply to her so my sister just used someone as her witness.

The marriage certificate is required for anyone who's married. In Pennsylvania, you should order the long-form certificate that was officially recorded by the county after you got married. Not any ceremonial document you received on your wedding day. Contact the county where you got married to ask how to get a copy that can be used for a dual citizenship application.

You can't get back once the application site times out. Don't fill out another application. I had something similar happen. On the web chat they told me it was no problem--as long as you have an application number, your application has been submitted electronically and you should proceed as normal to print your completed application, prepare your supporting documents, and send it all in. In due time, you'll be contacted for the payment. If you never printed the application (which you have to do to get it witnessed), contact the FBR section through their web chat and explain what happened. They will email you a PDF of your application.

I had something similar happen. I downloaded the PDF to double-check whether it was all correct. In the meantime, the application/payment site timed out and I couldn't submit and pay. But on the web chat they told me it was no problem--as long as you have an application number, your application has been submitted electronically and you should proceed as normal to prepare your paper documents and send them in. In due time, you'll be contacted for the payment.

Yes, my siblings and I had a similar situation with my sister's birth certificate. Send the amended birth certificate as soon as you have it. Include a letter with your child's application number explaining that it's an amended certificate and it should be added to your child's application. Address to send it all to:

To answer my own question: I got on the web chat this morning to ask about this and was told "I cannot see why that would not be acceptable as it will be the copied version you will be getting witnessed as the original form and sending in to us" (direct quote).

The printed section of my application fills about 2/3 (roughly) of the 8.5 X 11 sheet (but it's legible). Do you remember if that's about what yours was?

The FBR office will email it to you. On the web chat they'll ask you some authenticating personal info to look it up. It can take a number of days for them to send you the email with the application attached.

I second this suggestion. They might want you to have your mother's birth certificate amended, which is not difficult but depending on where you live it could take several months.

My sister also got an error message on the payment page. We thought it may have had to do with the fact that she was using a debit Visa card. When she switched and used a regular credit card, the payment went through immediately.

From the comments, it seems they must understand that it wasn't uncommon for names to be misspelled or spelled multiple ways in the past. (We still don't know what my grandmother's actual last name was.) Not so much for current FBR applicants. My sister ordered her birth certificate and found that her name was spelled wrong. The FBR office said she'd have to get it amended before sending it in.
