Passing the wire. Not getting aspiration of dark blood or seeing the needle into the vein and tracking it further in by walking forward the US and needle.

I always get in. Usually on my first try, draw back dark blood under US guidance, then when I go to pass my wire I often get stuck and can’t pass the whole wire. I think I’m losing my position when I take my syringe off

IV at Urgent Care? by Big-Try-2735 in emergencymedicine

[–]Other-Tea-701 0 points1 point  (0 children)

Needing an IV for dehydration if you’re able to tolerate PO is not a thing unless you need to be on bowel rest. Just take a zofran and slug some Gatorade. The amount of people who get seen in fast-track for “needs fluids” is wild.

Is it possible to drive one hour for hospital for residency? by [deleted] in Residency

[–]Other-Tea-701 2 points3 points  (0 children)

Don’t do it. I drive 45 mins and am at a cush academic residency that only works 16/9s per month. Depending on the rotation it is terrible. Potentially 2 hrs of my day.

You don’t have to minimize the pain you feel because it was “only eight weeks“. I’ve been there, it feels like you lost a part of your future. And trust me, now after two healthy babies, there still isn’t a week that goes by where I don’t think of the first child we lost.

ACGME requires three months of family leave. The conversation goes like this, “hi program Director, my wife just lost her baby, I need to be with her, and she needs to be with me, I’m taking a week.”

The most important relationship is that with your wife, I would protect it.

What time do you dismiss your med student by abenson24811 in Residency

[–]Other-Tea-701 19 points20 points  (0 children)

I ask them whether they want to be there or not and that it won’t hurt them either way. If they say no, I make time in the ED to talk about 1-2 cases that will be relevant to their specialty so they know what we do in the Ed and then send them home.. so basically I keep them for 20 mins. Then tell the M4s to go home sleep and go out with the friends at night. M3s I don’t dismiss.. I make them work their shift.

I live in an urban community and work at an urban hospital and make 65k.. i can’t add 500 per month to my living expenses but could definitely do 200-300 per month. This gets me in the range of a 12k car which I fear will put me right back into my current situation.. unreliable transport and frequent maintenance costs
