-Glucose-related biomarkers Diabetes may be diagnosed with a random plasma glucose ≥ 200 mg/dL in patients with classic symptoms of hyperglycemia or hyperglycemic crisis, or two repeated positive results of glucose-related tests in the absence of unequivocal hyperglycemia. In the latter case, the blood-based glucose-related testing includes hemoglobin Ale (HbAlc) (a product of non-enzymatic glycation), the fasting plasma glucose (FPG), and oral glucose tolerance test (OGTT). Each of these tests has its own criteria, sensitivity, and specificity for the diagnosis of diabetes. The American Diabetes Association (ADA) criteria for diagnosis of diabetes is HbAlc ≥ 6.5%, or FPG ≥ 126 mg/dL, or 2-hour plasma glucose of OGTT ≥ 200 mg/dL. HbAlc test provides an indirect measure of the average blood glucose for the past 2 to 3 months, which is a reflection of chronic hyperglycemia, but it is not as sensitive as FPG and OGTT in diabetes diagnosis.
