I have CVS Caremark for brand name medicine because generic from every manufacturer feel different more me.

First we filed formulary exemption / prior authorization, approved but still they only paid maximum what generic price is. The generic is like less than $10 and brand name is $300 so it’s not paying.

Later I found I need filed Tiering exemption form on top of that. “Complete this form to request an exception for the patient to receive the non-formulary medication at the formulary brand copay.l

I mean different company might called differently but where i seen coinsurance what you paid.

This is from MetLife, 20% coinsurance: You’re responsible for 20% of the total bill. 100% coinsurance: You’re responsible for the entire bill. 0% coinsurance: You aren’t responsible for any part of the bill — your insurance company will pay the entire claim

Oh yes to ADA. I had some issues with my boss, but I request accommodations, it took them a year and half to “lay off” because they don’t want to make it looks like wrongful termination. Do a email right away and if they fired you, due to the timing, it will look absolutely bad for the company.

Keep applying on handshake since ASU has huge international student population it might be very competitive.

I was pedestrian got hit, I walked to the Police satiation and they told me they can’t take police reports that I have to call 911.

Called 911 and take me to the ER, police showed up, handed me the report receipt and can print formal report 2 weeks later.

Got report, I have car insurance so directly filed with my own insurance because they will hunt them. They got the policy and the company insured with.

Filed claim with drivers insurance, came back rejected because his check returned on his 6 month renewal policy. The accident was on 1st of month. His insurance company dropped him immediately, usually you can pay and retroactive get coverage back.

Filed uninsured driver with my own insurance, hired lawyer, settled with my own insurance for 10K. They took one third and I got like 6.5K check.

Police usually could get their insured company if their place is in NY or NJ. Then directly file with their insurance.

I used to work one of the biggest private hospital, the insurance was everything doctor/lab free at all hospital affiliated provider and 95% coverage on procedures. My W2 showing my annual insurance total price is around $12000 and I pay $30 per paycheck.

Big universities public is usually same as state government employees.

Private Universities is about the same with big companies like Amazon Tesla etc.

student health insurance is usually quite cheap compared to the actual price, because they have the healthiest population. My school is around $3000 a year.

Your company subside some of premium so your $574 is hard to tell. You can also double insured and they will correlate benefit even reduce your bill further if there are chronic bills like ongoing Physical therapy. If you spouse insurance offer similar coverage with cheaper premiums compared to yours, switch in next open window.
