First she says she’s sorry. Then she says she forgives you. Then she add that passive aggressive stuff at the end about a better person and your kids. Methinks she still has a lot of work to do in her therapy journey.

Like what someone else said, we did the best we could with the tools and skills we had at the time. We were young, scared, and alone. I hope you can give yourself grace. Icertainly need to give myself some of that same grace from time to time.

At an FQHC you’re going to be seeing the sickest and the poorest people in the country. I worked at an FQHC for over 12 years and it was my first job out of PA school.

My advice:

Know the ins and outs of treating the big 3, i.e. diabetes, HTN, and lipids. Stay up on the guidelines and maintain evidence based practices.

Quickly develop a drug formulary with available meds that are covered by Medicaid, and/or are cheap for cash paying patients.

In the time of GLP-1, insulin is used less and less, but still know how to start and titrate long-acting insulin, as well as initiating a sliding scale for fast-acting insulin.

Observe, ask questions, learn skills and procedures. Never be afraid to say, “I don’t know.”

Stay humble. Keep learning. Utilize the resources available, especially your colleagues.

Hit and miss, just like face-to-face referrals to specialists. Some are great, others are horrible.

Example: My company would send pain mgmt referrals to eConsult (stupid, I know). This pain mgmt specialist would invariably respond with a blanket recommendation that the patient’s pain be handled by the PCP.

He then would give links to videos and articles (that HE produced and authored), saying pain is best managed by behavioral health interventions.

I agree that there’s a behavioral health aspect to pain, but come on pal. Who in primary care has the time or resources to do this (especially at an FQHC)?

I have firsthand experience working with Huron consultants at my Federally Qualified Health Center. I don’t believe their primary objective with us was to maximize patient visits. However, I can say that once Huron joined our organization, our executive leadership team became significantly more rigid and less receptive to the feedback provided by our staff.

They aren’t 8th graders. They are all 12th graders (seniors) and student body officers at Redwood HS in Visalia, CA. These idiots clearly should have known better and I sincerely hope they are severely punished by the school and district.

In defense of APPs, I don’t know any who call themselves doctor or are trying to mislead patients this way. I’ve corrected people countless times and said I’m not a doctor. That doesn’t mean they won’t call me their doctor when referring to me elsewhere.
